Tips for poaching an egg?

Just can't get it right no matter what I do tried everything!

What am I doing wrong?

Water not too bubbly and really really fresh eggs.

I think being able to properly poach and egg and properly make an omelette are signs of being a properly good cook.

I suck at both.
