Please tell me other people have made expensive mistakes at work?

So, I might have just been involved in a very expensive mistake at work. It wasn't solely my fault, it was a mixture of miscommunication and lack of understanding (supplier).
I had been dealing with something and the final evaluation was to happen while I was on annual leave. Apparently my coworker didn't really know about it (despite me telling them) and the suppler got the wrong end of the stick. At no point did anyone tell me there was a problem until today.
Now it's going to cost us quite a bit to fix it and I feel awful.
Have checked all my correspondence and it's all very clear, but could be misinterpreted if skimmed I suppose.

Please tell me other people have accidentally (and indirectly) cost their company chunks of money??

I was glazing a pair of spectacles in my first job (so cutting the lens to the correct shape to fit the frame). It was a really expensive lens that took 2 weeks to arrive from the lab in Germany.

Anyway, I broke it......re-ordered, apologies to patient and then did the same thing 2 weeks later. We sent those jobs away after that.

I once ordered an entire lorry full of Walls Ice Cream which I/and my manager thought was on promotion and wasn’t. It cost our business £8k but it wasn’t all my fault.

I once broke a toilet on my first day in a job and told no one. It was the only toilet for our office.

I once got done by an online scam (fraudulent emails) and transferred £30,000 to an incorrect bank account form my company's bank. It took nearly a year but we did get it back and I (amazingly) didn't get the sack. I was ill with worry for a solid year and will never forget that hideous feeling as it dawned on me what I had done and the powerful wish for the "Undo" button to be a real thing in every day life!
