So, story short, had police involvement with ex, need to take him off collections list for DS but they need something in writing.

I feel absolutely stupid because I have no idea what to write? And is it okay handwritten?

Can someone please tell me how it needs to be worded? Blush

I think handwritten should be fine - how about:

Due to an ongoing situation, which has merited police involvement, I would like to request that X is removed from the list of those able to collect DS.

Or if you don’t want to reference the police: “due to an ongoing situation, I have been advised to remove X from the list of those able to collect DS.”

I would address the letter and even speak to the Child Protection Officer at the school (usually the head) who would distribute the info to the necessary parties.
Office staff should not be given the responsibility for this and a teacher can mess up.
It might sound a bit OTT but your bases and child are covered
