Advice needed about my hurty thumb please

You know that terrible pain that you get when you accidentally stab something down your thumbnail? I have that but huge. I don't remember stabbing myself but jeez the pain is incredible. I've had to get up in the night to take painkillers.
My whold thumb hurts and the top feels number but painful.

It's been like this since about 8pm last night.
What do I dooooo? ?????

Is your thumb infected? I remember having a ragnail which got infected and the pain was like nothing I had ever felt before! Checked my thumb, saw a little bit of green, pricked it with a sterilised needle, significant amounts of puss came out, and I experienced instant relief!

No sign if any infection. No hangnails no swelling, no significant redness that I can see.
Just excruciating pain!

soak in as hot as you can stand salt water. (teaspoon to a pint)
