DD's keyworker accidentally threw a shoe at DD's head

If this happened to you, what would you do and how would you react?

I emailed a complaint to the nursery manager 2 days ago and I haven't even had a reply

It depends how old the child is I think, if they are 6 weeks old and got hit by a carelessly thrown shoe I would probably be a bit upset as that would be throwing quite recklessly. A child that can roll, crawl, walk not so much.
