Stupid question alert - what's a covering letter?

I need to send some evidence about being exempt from prescriptions to the NHS with a "covering letter".

What's that? Blush just a letter saying "Here's the proof you asked for"?

Include any relevant reference numbers in the letter too

Basically yes!
Make sure it lists everything you are sending so that they know what should be attached.
Send it to the person/address they say, and start the letter 'Dear the person'
Include any reference/case numbers already in use.

Pretty much yes. So you don't just shove the stuff in an envelope for someone to open and say WTF is all this and why have I been sent it? You put a in letter with your address and contact details at the top that says I am you asked me for I have enclosed , plus any other relevant info, yours sincerely
