It sounds to me like he has javelin arse.

For the last hour he has had a pain inside his bottom. I’ve suggested maybe piles? He said he also needs a wee but can’t go.

Do I have to take a look?

Is he on any medication? Urinary retention? Can be a side effect.
Has he had a poo? Could be cystitis, unusual in a male but not unknown. Sounds painful. Bath wee is a good idea

I would be worried about that for the reason that it might indicate prostate issues (though I'm not sure of the exact symptoms). He should get checked out.

He should go and get his prostate examined. Tell him to try to hot bath for a pee. How long has it been since his last one?

How long has it been since he last peed? I’d be worried about the retention if it’s been a while. Does he have any swelling/tightness at the front of his abdomen?

How old is he? And has he been drinking alcohol today?

To my ever living shame, we induced alcohol related urinary retention in my FIL after he tried to stay up drinking with me and my brother a few years ago. Blush DH’s trip to A&E with him in the early hours the next day was a little embarrassing, once the doctor explained what was going on - older man, large prostate, alcohol = inability to pee.

Probably a urinary infection - that can block up the waterworks and stop men being able to urinate, despite needing to go. A good sleep often helps but may need antibiotics.

Piles don't cause urination issues, in my experience.
UTI may give constant feeling of need to wee, but the tank is literally empty.
