Can someone help me with this Czech Republic address please?

I'm trying to return something and the company I'm sending it to have emailed me a returns label, I have to book a collection with UPS and on the website they want me to enter the recipients name, street address, city, postal code and phone number. I think the 'Turanka 106' might be the street address? Is the postal code the '62700'? What about the city? And I can't see anything that could be a phone number unless it's that 0044 number at the top

If you google that address and postcode it come up with the city Brno.
0044 is the international dialling code for the UK however so that might be a UK customer service line, maybe google the number and see what comes up? Otherwise email them and ask for a contact no for the return delivery.

If you need to do it in the UK format, I would probably put:

Name: C/O Moduslink RMA Dept
Address (line 1): CT Park Brno, Building 5
Address (line 2): Turanka 106
City: Brno
Postcode: 62700

But I would also stick the Czech-style address on the parcel.

Turanka 106 will be an area in Brno, like Westminster W2 in London.
