I'm going with mens' on the basis that it's a changing room for multiple men. I think.

How about just putting 'men' on the door? Even if you get it right some people will think it's wrong.

Men’s. Men is already a plural so the ‘s’ is only indicating the possessive, therefore the apostrophe comes before.

Male changing room would annoy me. Rooms don’t have a gender. Although I know this is widely used and it only annoys me a tiny bit. I know there are more important things to worry about.

Men's, Children's, Women's. They are already plurals so the appostrophe for possession doesn't need to move to the end to indicate plural.
