Puzzled by dates on fit note

I've been signed off for the past few months due to mental health difficulties. My GP issues a new fit note every fortnight/month, depending on how I'm doing.

My question is about the date section on the note. My last one stated:

"This will be the case for 2 months, or from X date to Y date"

Both parts were filled in, stating 2 months, plus a set date, which was for a single calendar month. Does this mean I'm covered for a full 2 months by this fit note, or does it expire after the date actually printed on the form?

I don't think they should fill both parts in, it seems like a mistake to me. To be on the safe side I would assume one month or query it with the surgery.

Yes, that's a mistake, only one part should be filled in. Did the GP tell you how long s/he was signing you off for? I'd phone the surgery and explain you need a new form ASAP due to the error. Workplaces can be difficult about those certificates so it's worth making sure it's totally correct.

Is it a computer issued one from your GP? that's a glitch, I'm a GP and the computer usually allows you to select only one option of from x date to y date OR for three weeks (or whatever)
