Trying to remember the name of this sci fi series I am watching (manned voyage to Mars)

Can anyone help me identify this series which I have started watch and now cannot remember the name?!?

It is a 2018 (maybe 2017) US tv series about a manned mission to Mars. In the first episode the rocket crashes (a malfunction caused by a coin in the mechanism) and the plans to go to Mars are put on hold.

The woman responsible for the mission, I think, has an English accent.

Cannot find it on IMDB despite having read the reviews about it on IMDB

It doesn’t have the word ‘Mars’ in it.

It’s quite slow moving - eg giving evidence at congressional hearings about reasons for crash

Altered carbon was good. But I did find it a bit heavy going.
I enjoyed Ascension, from a couple of years ago.
Defying Gravity is from 2009 but I rewatched it lately and it was as I remembered.
I have seen some really bad films in the last few months so I dont think I am the one to ask about that Grin
