No sleep!!! Desperate

Can anyone offer any advice on something a could be missing ???
My newly turned 10 month old has become a terrible sleeper...I can no longer put it down to cutting teeth, a cold , etc as it been to prolonged but all those additional baby things don’t help whatever is underlying .
He has disturbed sleep and becomes wriggly and cross which worsens through the night to the point where he thrashes so much a I can’t even cuddle him. I always offer water which he Often seems thirsty. Sometimes he passes wind, I wondered if it could he tripped wind but it’s every night . I have noticed that he is throwing his head back too when he gets into it and when I hold him. He also sticks his bum up and head down a lot and wriggle and thrashes right up the cot. This get worse through the night and be 2-4am it’s every 30-50 mins of unsettled no sleep. He had mild reflux as a smaller baby .... could this have returned ?? He is a great eater and a mild mannered happy boy in the daytime. Manages naps for an hour happily in the day. Seems like longer sleep we struggle with. I have even started giving him a feed in the night I case it’s hunger which he takes but doesn’t solve the problem. He has cut teeth , had ear infections and colds throughout but the sleep issue is a prolonged one so not sure it is related to those issues.
Any advice???
We are just so tired and I am feeling miserable and have an older son who is starting to suffer too.
Do I start reflux meds again or does he seem too old for this ?? Or infacol? It’s NOT just a clingy thing although I’m sure it will end that way if I can’t help him and me in the process xx thanks
