Why might a child do this?

My nephew, 10, academically solid but has had problems with social aspects of school. Does eye contact, is chatty, sociable and very confident. But also appears not to understand the consequences of his actions or care about them. Eg took a younger child’s water bottle and sprayed it and showed no regard to the child being upset. Makes very blunt/rude personal comments and doesn’t react beyond a bit of aggression/embarrassment when told off, and certainly no interest in how the other person is feeling.

He’s been referred to the paediatrician a few times but nothing conclusive ever happened. Could there be “something” or does he just need more discipline?

Well he's going to face consequences in the adult world for behaving badly. It would be good if he could learn now that it's nicer to be nice.
