Case description: A 3-year-old girl was referred to our pediatric outpatient clinic with breast development and vaginal discharge over a period of 6 months. The GAHT of her transgender father was estradiol spray 6.12 mg applied to both forearms daily. After 6 months this was changed to estradiol gel 3.75 mg daily for 7 months. The gel was manually applied to the chest, abdomen, shoulders, and thighs. The father reported skin to skin contact on a daily basis. A physical examination of the girl revealed a Tanner stage III for breast and Tanner stage I for pubic hair development.

Conclusions: Exposure to exogenous estradiol can lead to precocious puberty in prepubertal girls. Transgender persons should be thoroughly informed of the risk of transmission of transdermal hormones and be advised to wash hands, use gloves and avoid skin contact shortly after hormone application. Patients with children must be warned of the risk and Gender Clinics should consider the possibility of prescribing alternative routes of administration such as tablets or patches in high-risk patients.

I don't suppose they are going to get that sort of advice when they buy stuff on the internet black market that some "trans liberationist" has mixed up the bath or off some sex-offender who has been prescribed it as a condition of early release.

I think that it should be made clear that anyone using hormone gels should not do skin-to-skin contact with their child in that way.

This adds another gross and medically dangerous layer to the male weirdos who want to "breastfeed" babies, but yet the NHS says its fine 🤔

I wonder what exposure to these artificial hormones would do to a male child? Does anyone know? Certainly makes you wonder about these families with multiple trans people in them. Like, what are the odds?

Yes, you have to be incredibly careful with all hormone gels as they can be transferred by skin contact with others.

Yes. Also women using testosterone gel for HRT. (Though I would expect that woman to be perhaps grandma rather than mum). Stay away til you are all dry and clothed!

You need to leave it an hour before skin to skin contact. I specifically asked my GP this as I was concerned about DH coming into contact and at the time of starting HRT I’d just read an article on a man using testosterone gel for low T, causing precious puberty in his very young son.

I'm I'm on gel for HRT. I only use it when I know I won't be touched for an hour. I also use it on my stomach so it's not going to rub off on someone accidentally.

That’s a MUCH higher dose than you’d be on. I’m on four pumps - the maximum - which is 500 mcg (NOT mg which is what this guy was slapping on).

I’m interested why you think this father is a monster? The paper states that the daughter was inadvertently exposed, there’s nothing to suggest it was done on purpose.

This article is about a male using male hormones to address low testosterone. The advice to avoid skin to skin contact would be applicable to anyone using hormones not just trans people?

i think the paper could be more accurate to avoid confusion as it states he was applying 3.75 grams of gel which could realistically refer to the gel itself not the hormone.

Yes, of course the actual hormone content in the gel is much much lower, but as the gel (at least the one I use) comes in a pump dispenser and each pump is 1.25g (of gel not hormone) if the paper is actually referring to the gel this would equal 3 pumps would it not?
