Sudden bleeding after over two years of no periods

When I first started HRT 4 years ago my mirena coil replacement was inserted incorrectly, this meant I wasn't absorbing the progesterone, which in turn was causing thickening of my womb lining and causing clots and bleeding. I was prescribed progesterone tablets until two and a half years ago when a new mirena coil was fitted.

Apart from the few months when I wasn't getting the progesterone part of my HRT (I also use oestrogel) I haven't had periods for around 7 years. I think mainly due to the mirena coil, but maybe also just being perimenopausal.

Around three months ago I started spotting, especially after sex, which builds up for a few days before I pass a large blood clot. Things then settle for a few days before the cycle starts again. I was referred by the GP for a scan to check my coil was still in position, which it is, but now the GP has just messaged to say coil in position, no need for further action.

Am I wrong in thinking I need to find out why, after so long, I'm suddenly bleeding? Obviously it's probably just hormonal changes, but could be something that requires investigating. I did have a clear cervical smear test last year.

So, should I just wait to see if it stops, accept a leaky uterus, or ask for a follow up GP appointment?

Was the lining of your womb measured at the scan?

I've had a look on the NHS app at my test result, in short it just says:

IUCD in satisfactory position
IUCD obscures endometrium which cannot be properly assessed
No abnormality detected in either ovary
Trace of fluid noted at uterine fundus

So I guess all looked okay, but I'm no expert

Hello @nineteenpercent. Your thread just popped up and I wondered if you’d got this investigated further? I’ve just posted about similar spotting, which I’ve had on and off for the last 6 months.

Hi @Suffolker, the GP never did get back to me! My spotting did actually stop for a couple of weeks after I posted this thread, I passed a large clot, then things seemed to settle, so I was thinking maybe a cyst? Then last week I had what seemed like a proper period (first in 7 years). The bleeding has now stopped again, so I'm going to wait to see what happens next month. I'm putting it down to yet more hormonal changes, but if the bizarreness continues I will attempt to contact the GP again.

Your symptoms do sound very similar to mine, and I think if your spotting persists you should definitely push for further investigation.

Sorry to not be of more help, healthcare does seem to be severely lacking when it comes to such issues!

Thank you @nineteenpercent. I hope things settle down for you! It’s frustrating not to have any clear answers. I felt I was slightly robbed off with ‘come back in 6 months’, in case it’s something serious.
