Do you stick strictly to Mounjaro instructions?

I have been on MJ since December 2024. I took my first golden dose this morning having been generally quite risk adverse and sticking to the rules. I’m curious to know what other people do, and whether they stick strictly to the instructions, or have any other ways to increase the cost efficiency of their pens.

After necrotising fasciitis I have recently taken the ‘extra golden dose’ under couldn’t care. Until the new pens come in, if you are ok medically ok to tell me a condition in Nov 1991 diagnosed doesn’t exist then stand up to the podium.

We all need to be a little more careful when ordering given some pharmacies are sticking to a six week reorder period partly to cut down us mucking about with doses. You need to be prepared for a pharmacist to knock you back to 2.5mg if they feel it has been too long since your last order.

I’ve never used the remaining liquid, however I do count clicks to 9mg and jab on day 6. Different strokes for different folks.

Clemdfandango · 04/04/2026 16:54

Can someone explain the counting clicks thing to me please?

I've done 4 doses of 2.5mg and am about to start 5mg today.

I wouldn't have a clue how to use the golden dose either (call me thick!) 😳

It’s exactly what it sounds like.
60 clicks of any pen give the full dose stated on that pen. So 30 clicks of a pen gives a half dose eg 30 clicks of a 5mg pen gives 2.5mg dose and any fraction of 60 clicks can be calculated.

The ‘golden’ extra dose has long been the significant amount (more than a full dose) of extra liquid left in the kwik pen, only accessible by either breaking the mechanism or using a separate insulin syringe, but if you haven’t done it before, there’s not much point in learning now as it’s being phased out from this month onwards. Although I do think having back up syringes is a good plan for the rare cases that pen mechanisms fail. Saves arguing/waiting for a replacement if you can just get the dose out.

I did at first but as I’ve got more confident using it I’ve adapted it to suit my needs better and use it in response to how my body responds to it. If I have a lot of suppression one week I’ll take a few less mgs the next week to make the pen last longer, if I’ve got a holiday coming up I’ll skip a week as I don’t want to restrict myself on holiday. I always use the golden dose too

CreepyCoupe · 04/04/2026 17:29

No, when I was on it, I used the extra dose with a syringe every time. I also only injected every 2 weeks, sometimes 3.

Binus · 04/04/2026 17:33

I have been bonus dosing since very early on, and also now titrate.

Ah, I see.

I didn't realise 60 clicks gave a full dose. Thank you.

I've always done my own thing wuth it. I'm a midwife, so have no concerns about drawing up medication and calculating doses. I won't use a pen longer than 30 days though.

cortex10 · 04/04/2026 21:44

My supplier notified me this week that they are phasing in the new style pens this month - they no longer have any allowance for a ‘golden dose’.

Onegramatatime · 05/04/2026 01:40

No, I don't.
I had marked appetite suppression, i could barely eat 200 calories/day, i increased to taking it every 9 days.
I realised it would be much more financially efficient to buy the larger dose pens (even more efficient with golden dose). So have some pens that i will use for up to 16 weeks.
Decades experience of medicines, needles, syringes and calculating doses.
