Where do I start???

Hello! I’m finally finally hoping to start my weight loss journey. I have had a baby who has refused to come off the boob for ages! So I’ve been unable to start WLI as it’s not recommended while breastfeeding.

DS is now almost fully weaned so I’m starting to think I’ll be able to start in the next week when he’s completely off!

Where and how do I even start???

I have read mounjaro is better than ozempic i think…? But beyond that, I am not sure where to start.

The main priority for me is safety and ensuring I’m not risking my health as I am anxious about not being around for my kids (extreme, I know - but I have a lot of health anxiety after a bad pregnancy)

I don’t mind paying extra - I just want to make sure I have proper medical support and supervision throughout rather than doing it myself.

Any suggestions?

It's a choice between Mounjaro and Wegovy if you are in the UK. Wegovy is the same medication as Ozempic but just branded differently for weight loss and is what is available as a private prescription.

Have a look at monj.co.uk for a price comparison list of online pharmacies that are all GPhC registered and regulated. Some pharmacies offer additional support which is usually AI chatbots, apps, sometimes access to video calls with a pharmacist etc. The Monj website table has a column that shows which pharmacies offer additional support like this. They are often also subscription services which charge you every 4 weeks for the next prescription, so check what you are signing up to and how to cancel. After the initial first order where you can get a new customer discount, the prices can rise steeply. All pharmacies will offer Mounjaro or Wegovy, you can choose which you prefer.

If you want more interaction than apps and video calls, you could try a local independent pharmacy, as they often do weight loss appointments where you get weighed each month and maybe other checks too.

Mounjaro is more effective than Wegovy, but is much more expensive. Both are safe regulated prescription medications that have been tested and approved for treating obesity. They are not dangerous, any more so than any other prescription medication. Don't forget that being obese is itself risky!

I hope it's okay OP, but I have the same question as you in a way

I don't want to go to the local providers because I don't want to bump into anyone I know and to be honest, I don't trust them not to gossip about it. Everyone knows everyone here.

So looking at online things, I sent some queries to a few of them and got no replies yet and I'm guessing they will want a video so that's the first time you get to talk to them after you've ordered I don't know

They just want you to order and probably have no medical back up so maybe I do have to go to a local clinic I don't know how much extra they charge, but I would like to have some supervision and a person to talk to not a chatbot

Might drive to the next town over, but maybe everyone's doing that 😂

You will see prices for Mounjaro on monj.co.uk and Wegovy on sister site igovy.co.uk. Both only show GPhC pharmacies. There is also an option looking at smaller local pharmacies.

As you have already read, in trials, Mounjaro was the better medication. If price is not a barrier I would go with Mounjaro.

If you are using an online pharmacy, once you place an order they will do a consultation with you. Some are more thorough than others, but if you have any medical conditions or are not eligible for the medication it should get picked up at this point. You will also need to send in photos, perhaps do a quick video consultation to get your height, weight to confirm BMI, show photo ID. You are unlikely to talk to them unless you have an issue.

The pen is easy enough to use if you follow the instructions, watching a pharmacy produced video is also useful to see how to use it before your first time. Stick to the instructions until you understand what you are doing.

If you get unpleasant side effects (nausea, constipation, diarrhea, acid reflux etc) you should be able to speak to your provider by email or phone, but responses can sometimes be a day or two, or you can speak to your local pharmacy for support on how to manage them. Online pharmacies who specialise in these medications are likely to have more knowledge of them.

If you have a concerning, or urgent medical issue (pain etc which is rare, all medications have uncommon side effects) your provider who whether local or online only works office/pharmacy hours most of the time would not normally be your first point of contact, it would be the usual NHS services.

Other things you need to know? Most you will know from previous diets that you couldn't stick to! Mounjaro does not directly cause weight loss your calorie deficit will (work out your TDEE). To work with Mounjaro you will need to change how you eat - most eat protein focused meals (to preserve muscle), lots of veg/salad, as few UPFs as possible, reduced carbs, minimal sugar/sweeteners. Mounjaro makes it easier to stick to this long term (I am 16 months in and still don't miss or think about snacking etc). In the beginning it may give you appetite suppression, and you could find it difficult to eat, but that tends to wear off and you are left with reduced food cravings and portion control (if you continue to work with it). Losing weight fast is not healthy. ChatGPT is good for help with menus - use an app for the first few weeks to check your calories and macros.

Most of the (non medical) things you might need to talk to someone about, you can get more experiences from forums like this or if your provider has one their own chat forum, or perhaps reddit than from the pharmacies themselves. There are not many facebook groups anymore as facebook kept shutting them down.

@RoundandSad the online pharmacies will all have named pharmacists and some will have doctors as well, they will review all the online consultation forms and make the decision whether to prescribe or not. If they need more information from you they will contact you and ask for whatever info they need. You can also contact them and ask questions, they should state on their website how quickly they will get back to you, usually 1 to 3 days. As with any other prescription medication, if you have an urgent medical issue you would use your GP, 111 or A&E if necessary, not wait for an email or use an online chatbot.

I wonder what sort of supervision you were hoping for? An in person pharmacist is likely just to weigh you and that's about it.

They don't appear to have anyone named there and there are no prices listed either so why bother travelling

I suppose online provider then now I understand a bit better why everyone's using all these online providers

Do any providers let you start on 5 mg, I think that's what they do in America I don't want to spend a month getting used to it

@RoundandSad I have been on Mounjaro for 16 months and haven’t used 5mg yet - you might be similar, yes you need to start on lowest dose and then go up IF you need to. Many stay on 2.5mg for several months as it works for them.

@RoundandSad nowhere is likely to let you start on 5mg, it’s not the approved protocol and pharmacies in the UK would need to evidence robust reasons why they made a decision to do that, which they wouldn’t be able to.

Although 2.5mg is the starting dose many people lose weight whilst taking it, either straight away or at some point during that first month. It’s not a waste of time, and presumably a month is a small amount of the total time you’d expect to be taking Mounjaro?
