5th dose - Mounjaro

I have been taking MJ for almost a year and have never done the 5th/bonus/golden dose. I wanted to do things properly and follow the manufacturers instructions. But with bills going up and money becoming a bit tighter, I decided that it was probably madness to be throwing away what is essentially £30 of medication.

So I bought syringes and needles online and have just done it for the first time. I was terrified! For some reason, using an actual syringe seems a lot scarier than the pen. I was shaking and it took me ages to pluck up the courage to actually inject. It didn’t actually hurt but I really did not like doing it one bit. (I’m a little bit needle phobic but have been fine with the pens.) Now I’m paranoid that I might not have done it properly (I did measure out the correct dose.) Does anyone else do it this way and has all been okay?

If you have been on the medication for a year and probably on forums all over reading about the 5th dose you will know lots of people have done and do this and are perfectly fine.

What do you think you have done wrong that is worrying you?

I did the golden dose yesterday, with my first pen. It was really easy to extract and the only thing I didn't like was that the needle was a lot longer than the ones that come with the pen and I felt a bit squeamish injecting it. I figured if there was a full dose left in that expensive pen it was coming out and I was using it! Absolute no-brainer, quite honestly.

If you measured out the correct dose (0.6mg) of whatever pen you're using then there's no need for concern. The amount left in the pen is perfectly safe to use. I must admit I don't really understand why you'd be worried.

I'd rather overcome the needle fear than throw £30 a month

Mysticmaiden · 04/05/2025 19:52

I've done it 4 times with a syringe and 2 times doing the hard twist method. I actually have a 12mm fixed needle but I only insert it half way and I feel it less than the standard 4mm. If the fluid has gone from the pen then you would have done it fine. I've saved around £200-250 doing the 5th dose so it's definitely worth it! If you don't like the syringe method try the hard twist but it doesn't always plunge down. I use syringes for my NHS job so it's easy for me!

As PP have said, lots of people will have done this and been absolutely fine. If you measured out the correct dose and injected successfully then there is very very little that could possibly have gone wrong. I can’t think of anything that I’d be worried about.

I was massively needle phobic but have improved a lot recently, and have managed to do the 5th dose from the pen using syringes. I think using the pens for months has definitely got me accustomed to injecting and dealing with needles.

Seeing as you work with needles, can I ask advice on what to buy the 8mm scares me although I see you say 12mm felt less for you.

If I want to stick with 4mm will it be long enough to extract? Do I need to switch the needle after extracting and then injecting.

Is the following screenshot of my shopping basket the correct items to buy..... or should I just get the syringe with needle attached

I don't know if 4mm would be long enough to extract it, it depends on the thickness of the rubber seal. I use 12mm but I only insert it halfway. I've read even upto 12mm, it is still subcutaneous in fatty areas as the thickness of fatty skin is much thicker so should be fine whatever the needle size. I use the same fixed needle to extract and jab and not had any issues.
Mine cost £5.24 for 10 syringes from SHD medical including postage so that's why I went with them.

Thank you for taking the time to respond. I will do what you do and go with the 12mm x

I use 6mm (just because it is the same as my pen needle) and it long enough to extract. I haven't tried 4mm. I change needles between extracting and injecting just because it makes me feel more comfortable, many say you don't need to.

You could buy the 6mm to extract and 4mm to inject if you are used to 4mm and prefer it.

No need to go for something as long as 12mm, but if you do you need to think about your injection technique, much easier to stick to the smaller needles.

I use 8mm fixed needle syringes from UKMedi and find them very easy. In my opinion, it’s easier to inject with them than it is with the pen!!!

Apologies for the delay, I had a busy, tough few days! Good luck, hope they work for you. Look for 30-32G as they are finer and 1ml size.
