Psychiatric meds and Mounjaro

My DM has a long history of psychiatric illness and is on quite the cocktail of medication. Quetiapine, respiridone, lithium, clonazepam, along with drugs for diabetes, high blood pressure, cholesterol, thyroid, etc. She has been on Mounjaro since July (ozempic before that) and over the last few months we're seeing high levels of sedation (sleeping all night but still falling asleep in the morning), along with a general loss of function.

We took her to the GP yesterday and she's wondering if there could be some effect from the Mounjaro that is changing the way she's metabolising the drugs. She's going to chat to colleagues and get back to us.

Has anyone experienced changes if you're on psychiatric medications?

Who is prescribing the meds? I would wonder if the slowed gut transit means her meds need reviewing by a psychiatrist.

I'd be really interested in the outcome of this, as I'm on Lithium.

I couldn't find a pharmacy that was willing to prescribe due to this.

Psychiatrist and GP shared care. Psychiatrist has reduced the Quetiapine but recommended GP appointment. GP isn't sure if Mounjaro could be impacting.

I'm on olanzapine and fluoxetine. Started mounjaro due to weight gain from the olanzapine. been on it since September.

I'd doing Ok, have lost weight, finding the cravings much more manageable. A bit less anxious, quite tired though.

Might be worth getting blood tests for e.g. ferritin, vitamin d etc, electrolytes, how is her general nutrition? All can have quite an impact on health / mental health. My GP is pretty good and does these for me.

Ugh that's a pain then. Hard to know how to proceed if neither of them are clear on the side effect profile.

Yes, it's tricky. They're doing a full blood panel today. They've found that she has a UTI, so that could be contributing. It's a bit of a jigsaw!

Is an Endocrinologist prescribing the Mounjaro? If not, that’s where you are going wrong. Go and see someone like Harvinder Chahal at London Medical. He deals a lot with people on psych meds and weight loss meds as a result of the psych meds.

No, it was the GP who prescribed the Mounjaro. She's only on a low dose because the aim was to lower her HbA1c rather than to lose weight (although she is very obese). It has achieved that, so they aren't increasing the dose.

Thanks for the recommendation but going to see someone privately isn't an option and travel is very difficult anyway.
