Supplier refusing to prescribe

I started Mounjaro a few months ago as my BMI was 28+ and I had high blood pressure and cholesterol.

Fast forward to now, I have lost a fair bit of weight and my BMI is now 24.9 so just in the healthy category. I am probably carrying at least a stone more fat than I would like, and if I were to hit my target weight then I would have a BMI of 23. My GP and I discussed an appropriate target before I started based on what I weighed before my hormones went nuts and my frame which is small. My BP is down a bit, as is my cholesterol, but neither is where they really need to be for longer term health.

I am currently on 7.5mg but have had some issues with low blood sugar and vomiting so contacted my provider to ask that with the next pen I go back to 5mg. I also wouldn't mind losing a bit more slowly as this has all been very fast. Had I not done so they would have sent me a 10mg pen without checking on me at all.

Instead they came back and said that there is no need for me to continue so instead they are putting me on 5mg for a month and then down to 2.5mg and then off altogether within 6 months. I'm really annoyed as my BMI is only just healthy and they were not monitoring me at all until I made a request. Is this what all providers do when your BMI hits 25? Has anyone managed to switch providers when their BMI was in the healthy zone? I was hoping eventually to transition to a low dose and potentially stay on it forever so as to get the advantages of lower inflammation in my body but it seems for that I'll perhaps need to get an endocrinologist on board.

Lots of us have moved to other prescribers who have maintenance plans. Some of these will accept new patients who are already below BMI 25 as long as they can prove their previous prescriptions.

Thanks. Do you know if other providers would force me to move down to 2.5mg in a month or would they allow me to stay at 5mg longer term?

From what I've seen, 5mg is the lowest official maintenance dose, so staying on it should be fine.

2.5mg is officially a 1 month stepping dose between 0 and 5mg, although many providers let people stay on it for longer.

The company I'm with would have a video consultation with you and together, you decide what you want to do eg stay where you are, reduce dose gradually and come off or stay on long term.

I’d hate that, I can’t think of anything more patronising and a waste of time. I get some folks need that level of support, but for me, I’d rather come off them than have to deal with that every month.

That's fine. A lot of us, however, don't want to come off once we're at goal weight as we know we'll put the weight back on, just like every other time we've lost weight.

I didn't say it's every month either; it's when you move onto maintenance - the prescriber needs to know it's safe to continue to prescribe to you given that you may be at a healthy BMI, and ultimately they're answerable to the regulator about on-going prescribing decisions. I'm personally happy to have a source of advice and don't find it all 'patronising and a waste of time'. This is a powerful medication and to have some expert oversight is to be expected I'd have thought 🤷

But each to their own.
