Hey!

When I ordered my first pen i must have been really tired.... I ended up ordering the 2.5 from 2 different providers....

I have now ordered the 5mg to go onto next, but my question is re the "spare" 2.5 that I have.

I've seen on some of these threads you can split the dose on a higher dose pen to titrate down by number of clicks etc

Can i do the reverse and use my spare 2.5 with a double dose, to make it the equivalent of a 5mg dose?

I cant really think of any issues myself, other than fluid volume, but figured i could do 2.5 in 2 sides of tummy rather than all in one side?

Yes you can do two injections of 2.5mg to make 5mg. Your pen just won't last as long; your 2.5mg pen will only last you 2 weeks instead of 4 but you'll have to accept the loss.

mydogisacloud · 03/12/2024 13:15

@IMustBeInvisible perfect thank you! I just don't want to get caught into being a "pen behind" and have to plan ahead constantly!

I think I'd rather lose the two weeks and be on a level if that makes sense?

Yes I understand, @mydogisacloud .In that case, you're good to go.

HippoPortlyMouse · 03/12/2024 14:11

Just be aware you don't have to increase your dose if you don't want to. If you're getting decent suppression and are happy at 2.5, you can just use the second 2.5 normally, over a month. If you are ready to make the jump to 5 though, you can indeed double up the dose.

mydogisacloud · 03/12/2024 14:36

Thanks @HippoPortlyMouse - the suppression is good but waning a bit over the last week, so keen to try the 5...

just hopeful i don't start getting side effects; more than happy to stay where i am as long as it keeps working

@IMustBeInvisible thank you :)

You could take the 2.5 every 4-5 days so it lasts a bit longer,

mydogisacloud · 03/12/2024 17:33

@memememe how would that work with then switching back to 5 with a dose once a week?

OP posts:
memememe · 03/12/2024 18:42

I would just wait 7 days after your last 2.5 before you take the 5mg

WeAllHaveWings · 03/12/2024 20:08

I find suppression can come and go depending on how nourished your body is - I have been on 3.75mg for 19 weeks now, some weeks it is less, then other weeks more, you need to eat and you may find suppression is less because you haven't been eating enough, make sure it is within your calorie deficit but don't eat too much below or your risk health issues from rapid weight loss. So consider using your 2.5mg as normal.

It is also wise not to rush up the doses, especially if you have a lot to lose, as once you hit the highest dose you have nowhere else to go.

I don't see any issue with using 2.5mg in the way you are suggesting (except it is a very expensive way to do it). You could potentially look at a mid way dose to make your 2.5mg pen last longer and take you up to 5mg more gradually.

If you do 96 clicks (0.96ml) it would give you a 2.5mg + 1.5mg = 4mg. You would get three doses of this. It would need to be done over two injections from the pen each week (or use a syringe) - I would do another month of 2.5mg to avoid extra needles 🤣. The third dose would need a hard twist/or use a syringe to access. Always use a new needle for every injection, even if done directly after the first one.

Or do three doses - first one another week of 2.5mg and see how the suppression is, then the next two weeks 96 clicks for 4mg which will mean you probably don't need a hard twist or syringe.

Unless it is for a one off change of injection day, I would stick to the 7 day dosing schedule unless you speak to your provider first. Changing your dose is (imo) low risk, but I personally wouldn't mess with your dosing schedule without input from someone qualified, no one knows if that puts extra load on your liver/kidneys trying to clear spikes in the medication more frequently and it might not be immediately apparent the impact of doing that.

Alternatively if you have no issue keeping it the fridge for a bit you could use it to top up a 5mg pen when you are ready to go up to 7.5mg or for reducing back down to 2.5 if you find 5mg is too much.

Basically see how you get on on 5mg before making a decision

I've found 5mg to be too strong so will be going back to 2.5 mg

mydogisacloud · 03/12/2024 22:45

Thanks for the posts - really given me a lot to think about. Need to check the expiry/instructions in the remaining 2.5 in the fridge - didnt warm up and has remained in the fridge since it arrived, so it should be fine to holed onto for another month... right?

@WeAllHaveWings
Really great post, thank you!
This hard twist, is it literally just a case of...twisting harder? I think all i have left in my current 2.5 is the "bonus dose", and it seems such a waste to not use it

@Iudncuewbccgrcb
I like the thought of keeping it to increase to 7.5 after my 5 pan, or indeed as a safety net to reduce if needed...

OP posts:
WeAllHaveWings · 03/12/2024 23:33

@mydogisacloud this video shows the hard twist. I can never do it with my hands so have to use pliers to help grip it and then turn firmly but carefully

It has worked for me every time so far but many are now saying the dose knob breaks off when they try it and perhaps the pen mechanism has been changed to stop people doing this. The other method is to order some syringes to extract the medication through the red seal, a full dose of any pen strength is 0.6ml (google 8mm 1ml 32G syringes and buy from a medical supplier not amazon as you can't be sure they are human quality)

It is worth having syringes anyway in case you have a pen failure (replace pen needle to check it is not just a faulty pen needle first) and cannot be bothered with returning.

Check your expiry date. I’m in a similar position - checked and the expiry date is in August. I’m hopeful I’ll be titrating down before that so keeping it in the fridge until then

mydogisacloud · 05/12/2024 18:51

Amazing, thank you both @NoAprilFool and @WeAllHaveWings !
