The "what are we going to do" could tie in with John not being John. Maybe that's why he's fucked up at the surgery as he's not medically trained (surgery has firm for employing non qualified medical personnel) but the real John in the hospital bed is medically trained.

last I remember Mackenzie was working on the farm hence why he wasn’t in the garage but he can’t have gone missing and nobody noticed

Maybe John sent a text from Mack’s phone to Charity to say he was working away at some other farm or garage and will be a while. That’s why we haven’t seen him

Why on earth would John have it in for Mackenzie? Unless Mackenzie saw John with Nate's body as Mackenzie was good friends with Nate.
