I spend a lot of time in hospital. The majority of nurses looking after me are from the Philippines. All of them are supporting entire extended families back home. It is a really complex ethical dilemma. The NHS would collapse without them. Of course they are needed back home, but they come here to earn the money.

Yes, definitely economically the obvious choice for those nurses to take their qualification and make the best personal choice for them and their families. And the right choice for the NHS to attract in skilled practitioners who want to work for the offered salary.
But huge ethical challenges at institutional and national level.

I was looked after by a nurse from the Philippines recently....she had small children back home who she hadn’t seen for over two years! No idea when she will see them again because of covid. Really heart breaking.

One of the surgeons shown has just operated on my Dad. It's exactly as it's shown. The operations can't go ahead until there's an ITU bed to go to. Too much money is wasted on staff that aren't needed such as all the middle managers.

@Rillington what's a middle manager ? It's really not as black and white as that. Many are clinicians too. Our ICU consultant is director of crit care but also doubles as a consultant/anaesthetist. It's not like there are loads of clip board wielding managers loitering around.

It really frustrates me that people have swallowed this middle manager bollocks hook line and sinker. Sure, there are inefficiencies in the NHS, no doubt about that, but we need admin and non clinical staff - otherwise we clinicians have to absorb their roles and do less of our own!

We absolutely need managers at all levels in the NHS. They need to be trained and experienced in management and have some clinical background. Unfortunately that is a rare combination IME.
It is really frustrating being managed by someone who has absolutely no clue about the job/team they are managing. Ditto being managed by a clinical person who has no idea how to manage a team.
I was a band 7 managing and training my team. The most difficult part was the constant need to explain to the next level of management what our job actually entailed and why certain things they wanted us to do would put lives at risk. It was exhausting.

@endofthelinefinally I experience this working for the local authority, I frequently have to explain safeguarding procedure to my line manager who will then tell me to do what I think is best Hmm IME a lot of this comes from the constant restructuring and slotting people into roles where they have no experience of direct working with the client group. Is that similar in NHS do you think?

Yes. IME. You find the same people shuffled round different jobs/departments. It is hard to keep track. You find evidence of the Peter principle a lot. People get promoted to just above their level of competence. Sometimes a long way above their level of competence.

I was heartbroken for David and his family, he had so much to live for, part of such a lovely, close family. So upset for them.

I do hope Drew continues to be healthy.

It was heartbreaking to watch. The decisions that have to be made - and yet so many people have disregarded the pleas to protect the NHS. I also hate the blaming of the “Middle Managers” - everyone will be trying their best in a battle that just can’t be won

Catching up now. The poor girl with the bad hip was cancelled because they couldn't get a paediatric team. She's 23!! I'm a paediatric nurse and a 23 year old would not be getting a paediatric team in my hospital. And we do exactly the same surgery on a regular basis. Teenagers with special needs have to be handed over to adult services at some point. Adult services need to learn how to look after these people.

How come over 20's are treated by paediatric staff? Is it because they have long term conditions?

She was 23!? I thought she was 12 or so. I really like this programme, but it really does highlight how difficult it is to run a hospital during normal times, and just how covid has made things so difficult. I would not want to be any of those senior consultants fighting for whose patient is the most worthy of having the operation they so desperately need.

We know she had physical health problems from when she was little and learning disabilities it said

@purpleme12 oh yes, I know. I was just surprised she was about the same age as my ed. She did have a lot of medical conditions to deal with. I thought my dds do, but different things with different results.
