Can someone please read these notes ? I have no idea what the writing says 🤣

This is from a few weeks ago , I was measured and told baby bump was small, got to hospital and scan said baby to big

I met with doctor and he said baby measuring but no real concern (had this with my last, told me he would be massive and was 7lb 9 and full term )

The doctors said if I've not had baby by 40 weeks they would induce me, but i can't see that on these notes, but I can't actually understand a work of it!

Any help much appreciated , thank you

@SmartCar that's aboit all I could get as well 🤦‍♀️ and 1 word that says 'plan' the rest is a complete mystery !

It's terrible handwriting. Sometimes they have put a down arrow which is where I think it means "reduced"...

I think first line is saying something like "Scan .... ([due to?] reduced SFH)" (SFH is the bump tape measurement)

All I can make out is "Sum: [Growth?] on 90th centile" on 2nd line.

So that tallies up with what you said of measurement being small/reduced and then scan showing big.

On 4th line it might say "plan: HBAC" (did you say you wanted a home birth after a previous caesarean)?

And on penultimate line I think it says "- if reduced fetal movement/leak/bleeds/pain ..." presumably it's saying "do X if patient has any of those symptoms"

You should get someone to translate though at next appt (or sooner!)

It's appalling if you think about it....what happens if you are away from home and need urgent care related to the pregnancy. You've been diligent and have taken your notes with you but no one at the other hospital can bloody read them!!! 😅

Nevermind the fact that I believe all your medical notes etc should be available and comprehendible to you as the patient. 🙄

And sorry OP, other than 90th percentile I couldn't make out a thing.

That's just my guess. It could say something completely different!!

This midwife has doctor's handwriting, for sure!

Third line from bottom, could that possibly say "risks around [......] explained" ?

I think also after the first 90th centile on 2nd line it says "(AC on 90th centile)" which means it's the abdominal circumference that was on the 90th centile

They measure
BPD (head measurement/diameter)
HC (head circumference)
AC (abdominal circumference)
FL (femur length)

So my assumption would be, out of those measurements, it's the tummy that was measuring big xx

3rd line might also say "W nml"... "weight normal"?!? Estimated weight from scan was normal? I'm clutching at straws a bit now!

This is a puzzle 🤦🏻‍♀️😕 Not great though

ive never had a c section nor do i want a home birth so reslly confused about the HBAC! I can also make then letters out but no idea why it says that 🤦‍♀️

Would be much easier it they typed thr notes up and printed them !

HBA1C is a measure of your blood sugar

To me it looks like the 'plan' is to check your HBA1C and either repeat or routine bloods. I'd love to have a closer look at an upright version as I do this at work all the time and it's quite the challenge

Not sure how to turn picture around as on my phone it's the correct way, but flips when uploaded 🤦‍♀️ they did do blood tests for sugar which I now assume is the HBAC1 so that makes a bit more sense now!

Blood tests were clear

He did say baby was meaning at 6lb at 32 weeks , but I know how inaccurate this measurements can be, but it seems there it no follow up for baby being big 🤷‍♀️ last time I had a few more scans before baby was born when they said je would be big, but not this time

W nml - water normal?

Plan - HBAC?(GD test I think), or HBM?(heart beat monitor)

Can't read next line

I think it’s basically saying they will do an hba1c and review bloods, I assume if AC is high centile they will be wanting to check you for gestational diabetes, was this mentioned at all?

I can’t make out anything about induction so maybe this was mentioned as an option rather than plan?

Did they discuss risks of a vaginal delivery with a large baby with you? The penultimate line looks like it could say “shoulder dystocia, perineal tear…” can’t make out what PNLL could mean??

Then the final line I think says return if you have reduced fetal movements/leaks (waters)/bleeding/pain.

When is your next appointment? I wouldn’t be worried but I would be calling the day assessment unit to see if your hba1c results are back yet.

Sorry I can’t figure out any more, it’s bad practice really to have such illegible writing.

The plan I think is to measure HBA1C and random blood glucose. It's to check for diabetes which is sometimes linked to your baby being bigger. Are you 29w op? Because there is mention at the bottom of 31w/2w which might mean they are going to look at you again in two weeks. The handwriting is some of the worst I've seen and I have to look at this sort of thing a lot

Shoulder dystocia- bloody autocorrect!! I was wondering if that bit might be your history of a previous difficult delivery? Or maybe they were just logging that they'd discussed these risks of having a bigger baby with you?

It's 'lv normal' or liquor volume which is your waters. I can only assume this is a consultant as it seems the higher up the ladder they go the worse their handwriting gets!

Plan: hba1c routine bloods (??) Risks around ?? explained Shoulder dystocia, perineal tear, pph If rfm/leaks/bleeds/? in w31'

So basically baby is measuring on the 90th centile including the tummy measurement, waters look normal
Plan test for diabetes
Risks of vaginal delivery were (hopefully) explained to you which include shoulder dystocia, tearing and excessive bleeding.
If you have reduced movements, water breaking or bleeding to ??? Maybe go to ward 31 or maybe be seen again at 31 weeks?
