Nappy rash won’t go

Hello everyone,

My 6 week old has had nappy rash for 3 weeks. The skin is broken and looks so sore. It's only between his bottom cheeks no where else. Have tried, metanium, Bepanthan , sudocrem, Daktacort and timodene. No difference! Change nappy regularly and have nappy free time at least two hours a day. I have been doctors who prescribed some of the above creams and did a swab which came back with no infection etc. Have tried different brands of nappies etc. Can anyone suggest anything else?! It doesn't seem to bother him generally only when nappy changing time comes but it breaks my heart to see it!

Are you using baby wipes? I stopped and started using cotton wool and warm water then dabbing dry with a clean flannel / towel

I also gave oatmeal baths and stopped using any bubble baths / creams / baby powder at bath times - it got better after about a week I think

My son had awful nappy rash when really tiny too because he was bf and pooped pretty much constantly. The GP prescribed some hydrocortisone cream to be used very very sparingly but it did clear it up almost immediately. we were also advised not to use any wipes or water at all as it would dry out his skin - GP advised we use baby lotion and cotton wool to clean him up and that really helped too so would def recommend that.

We’d have that as a backup to use when it flared up and use a good barrier all the other times.

I remember feeling awful about it though as it does look terrible and must be so sore for them. Hope it clears up quickly!

I used the calendula nappy cream with my eldest and was amazing at clearing up any rash overnight. However it didn't suit ds2 so we used sudocrem with him.

Thanks everyone. I tried dropping wipes for cotton wool and water but no difference. I was using water wipes which I thought would be ok as they say they purer than cotton wool and water?! I have gone back to using them as I find them easier to clean with which tends to mean I am touching him less and drying off gently dabbing with cotton wool. It hasn't got any worse going back to the wipes and the open air makes it better but as soon as back in a nappy it's bad again. He constantly poos! Not massive dirty nappies but he's windy and follows through with little poos a lot! Never a clean nappy even if change after ten minutes!! Just at my wits end with the skin being broken and it spotting blood at times.

Have you tried yellow metanium? It’s amazing! It takes any soreness away after a couple of applications

Try Metanium mixed with sudocreme, don't pat dry with cotton wool doesn't take long. Are you breastfeeding? My DD used to get bad if I ate anything acidic like tomatos or oranges. Used to happen when she went onto solids too.

It could be few things..it could be the nappies which are made mostly from plastic, glue and lots of chemicals so it might be a reaction to those..worth trying the 'eco' ones which are less 'chemical' or try cloth to see if it helps..other thing could be reacting to something in diet (if you breastfeed your diet) or to dairy in formula where his poos are quite acidy and the contact with skin even if for a little time makes the skin react..I would try to change the nappies first to see if it helps..also Welleda nappy cream is great it helped us a lot where other creams failed..

A really thin layer of yellow metanium. Perhaps try rascal & friends nappies too - they're chemical free

Don’t use anything containing zinc on broken skin (no sudocrem). Get hold of some Cavilon spray from your GP or from Amazon. It’s a protective spray for broken skin. There’s also Cavilon cream which is a brilliant barrier cream but not while the skin is broken. The other combo that is great is yellow metanium covered with a layer of bepanthen.

And the constant pooing could be dairy allergy. This is how my DC’s symptoms showed.

Another vote for yellow metanium here.

Is he ok in every other respect? As my daughter had a bad rash that nothing would clear at that sort of age and it turned out it was linked to her as yet undiagnosed milk allergy and basically milk made her poo acidic and was burning her bottom.

Yellow metanium is ok but always ended in thrush for us.
Coconut oil has worked best.

DS only ever had nappy rash with sudocrem, water wipes and Aldi nappies, it was a process of elimination with the wipes and sudocrem and the nappies only became apparent when twice nursery put him in someone else's aldi nappy (we'd supplied our own but I think they got mixed up) , both times he'd been in them less than an hour and his skin came up terribly. Yellow metanium (not the usual one) was brilliant, once it cleared we used child's farm nappy cream and either rascal and friends, kit and kin or eco naty nappies and no rashes

Both my DCs had this; constant poos and awful nappy rash (broken bleeding skin) that wouldn't clear up despite trying various different creams. In both cases, it was caused by CMPA. Please consider whether your baby might have it - info here: www.allergyuk.org/information-and-advice/conditions-and-symptoms/469-cows-milk-allergy

Yellow metanium helped DC1 but didn't help DC2, we found the Weleda cream best for her. We did also get something on prescription from the GP, I forget what. It was cutting out dairy that sorted it though as she stopped pooing so much and her skin could heal.

Waterwipes are great and we still use them on our two year old, but we found that they are SO wet that they led to soreness if we didn't dry her off with a muslin after using them - might be worth a try. Yellow Metanium also is a godsend!

Another one to suggest it might be CMPA. My poor DD was the same - absolutely no cream would even slightly improve the blisters as she basically had poo leaking out of her bum at all times. Sad

I cut out dairy from my diet and the rash cleared up very quickly.

We now use Weleda calendula nappy cream if she gets a bit of nappy rash and it is absolutely amazing - gone within a few hours - so I would suggest that one as a maintenance cream once you get on top of the root cause.

I absolutely swear by Waleda Calendula cream, literally the only thing that cleared by little girls nappy rash up

My baby was the same. It was so stressful at the time, I hope you are ok.
We used ilex, contacted the company direct and they sent me a sample tube for free. It was brilliant and really was the only thing that worked, as no other creams would stick. Cleared open sores within 48 hours.

Thanks everyone. I have been using yellow metanium since it first started showing with no luck. I did wonder if it was some sort of allergy, but he doesn't seem unwell in any other way. He has bad reflux but everyone says that's normal for babies (I'm a FTM!). I'm combo feeding breast and formula rough equally due to baby being premature and my supply being low/latching problems, aiming to breastfeed exclusively but not got there yet. Will try some of the things you have all suggested, hopefully something will work for the poor little guy!

Reflux is another sign of CMPA.
Nappy rash and reflux were the two main symptoms for us.
Has baby been assessed for tongue tie by someone properly trained to do it?
That can often cause latch problems and can also cause reflux too.

@NameChange30 hope you don’t mind me reopening this feed but I’m at my wits end. I’m on week 6 of having nappy rash with my newborn and feel completely cheated from my newborn experience. I’ve tried absolutely everything nothing wil heal it - I’m a children’s nurse myself and have resisted to using a barrier cream we use on babies going through chemotherapy and even that isn’t healing it - 3/4 hours of nappy free time a day too. It’s relentless. I’m considering whether or not now it is a cmpa allergy - as this nappy rash alongside a few other rashes on his body (behind his neck, comes out with hives on his tummy - hard as it’s also peak summer and defo has sensitive skin but something isn’t right and I’m at my wits end)
did you find the cure/cause?
