Baby with extreme spit up, desperate mum

Sorry this is long...

Hello all, my 4 month old ds has terrible spit up since newborn... he has never been able to sleep on his back due to this... he will easily spit up 8 times after a feed and alot of the time its hours after he has eaten as well. Sometimes big amounts sometimes small, he is on Omeprazole... It has been mostly a "washing problem" as he hadnt been in discomfort as much up until recently.

Recently it has been awful, so much vomit, and it is affecting his daytime sleep, he is waking up during every nap crying and in discomfort then vomiting. He vomits when completely upright, sitting, lying, all positions.

Has anyone experienced this and found anything to help? I have been reading about "nat phos" from Holland and Barrett, has anyone tried this with any success?

Sorry for the rant, im just feeling so helpless as I dont know what to do to help him, would be grateful for any suggestions or experiences.

Also should mention he is breastfed and only fed sitting completely upright.

Does his dose of omeprazole need to be increased? It is weight dependent, so ask your doctor if he's on the best dose for his weight.

There is also another medication I think, that speeds up the rate of stomach emptying. DS was never put on it, but I've read about it. Maybe someone else will be along to tell you more about it.

Have you tried carobel? It's a thickener, you can give it to him on a spoon before a breastfeed or mix it into a bottle of expressed milk. It weighs down the milk in the stomach.

Introducing solids at 17 weeks really helped DS. Between omeprazole, carobel, and solids, we really got things under control at 4 months.

I know how awful it is though, and I really feel for you. I would go back to the doctor and say omeprazole isn't doing the job, and I'd keep going back until they found a solution (in fact, it's what I had to do for DS but we eventually worked everything out). Good luck.

@PippinStar Thank you so much for your helpful response and taking the time to reply to me.

His dose was 1/2 a tablet but he vomits alot of it back up so we increased to a full tablet so that he might even get a little of it but again just vomits most of it up so I thunk part of the issue is that because his vomiting has increased again hes getting less of the medication so the discomfort is back.

I have spoken to health visitor and she recommended starting solids early too... I really hoped to wait until 6 months but I may need to try earlier, for everyones sanity lol

No, I had not heard of it but I just googled it and read some reviews on amazon, it sounds like magic! I will be ordering some today. Is it the "instant Carobel" one I need? Trying not to feel too hopeful just incase lol.

I had this with my daughter from a few weeks old. The only thing that worked was Carabel thickener in her milk. I had to use a lot more than recommended and had to use size 3 teets but it solved the problem and she was much more comfortable

That's the one - Cow and Gate Instant Carobel. It's not a miracle, but it is really helpful!! The paedetrician we saw initially swears by it, insists on it for all her reflux patients.

It can make them a bit gassy and either loose stools or constipated until they get used to it, however, so you may need to play around with the dose and increase gradually. I bottle fed so not in the same boat as you, but I used less than the recommended dose and left it longer to thicken in the bottle. Even now DS is 14 months and we still use it!

Maybe when your little one starts solids you can dissolve the tablet in a tiny bit of warm water and mix with the solids - that's what I did. He might be able to keep it down then as the solids will weigh it down.

Oh yes, as pp said, if using bottles of expressed milk with carobel you will need to go up a teat size or two!

As above, just came to say have you tried cutting out dairy? If he’s ff you’ll need special formula, if bf you’ll need to go dairy free yourself for a minimum 6 weeks. My second daughters main symptom was just puking all the time - she had a slightly rashy face and stinky farts but those things weren’t as noticeable.

@PippinStar Thank you for all the tips! I got it from my local chemist this evening, tried the spoon feed method mixed with milk but he spat it out so I will express next feed and try it in a bottle.

@2blessed2bestressed I see why it would need a larger teet, I only did 1 spoonful and it was so thick, thank you I will get one ready for tonight

That's great @candystar ... hopefully he will take it in the bottle. (You can take a needle / knife to a teat to widen it slightly if necessary!)

No I haven't tried that, when it initially all started my health visitor said that food intolerance would be rare for a b.fed baby so not to change my diet.

I would be happy (well apart the chocolate 😄 ) to try to cut out milk though if there is a chance it would help him.

My first was like this. Would literally vomit up half a bottle after every feed.
We tried everything and in the end weaning early was the only thing that helped. We weaned at 17 weeks.
The other things that helped a moderate amount were changing to hipp organic milk from cow and gate, and the mam anti colic bottles.

My DC2 was like this and it was such a tough few months - you have my sympathy. Cutting dairy did improve things for us.

The dairy thing is quite common I think - it’s worth a try anyway. There’s a really good Facebook group called breastfeeding with allergies or something like that which gives you everything you need to know, and crucially has a list of the nice food you can still have (turns out milk lurks in everything!). My daughter did the milk ladder at 12 months and raced straight through so it’s not something that lasts forever - they often outgrow it early on.

About 10% off babies have CMPA (cow milk protein allergy) and yes it does effect breast feed babies too. I would join the Facebook group breastfeeding- CMPA support. I would cut out dairy and soya for 6 weeks and see if it makes a difference. You will need to check all labels carefully and make sure you get enough calcium and iodine.

Sorry for late reply, we had family illness going on this last week.

So I was reading up about the cows milk and read something about caffine too, i have cut my caffine down to 1/2 teaspoon of coffee a day, he has been a bit better the last few days but not all of the day so I think that was just coincidence.
I have also been keeping a food diary but I cant find any common denominators with bad times.

@ElsieBobo Im going to buy non dairy milk and cut out chocolate, cheese and yogurts too and see if it helps. Any tips on what to buy? Is almond milk nice?

@Crabbo I may start a facebook account to check that out. So glad to hear it hasnt lasted forever for you.

Thank you @Selfsettling3 Defo a facebook acc is required. I eat alot of cheese for my protein intake too so will need to try to find substitutes

Also, the strange thing is he goes to bed at night and sleeps right through and doesnt vomit.
Would this be typical of a cows milk intolerance for it not to effect him in his sleep or to only effect him worse at certain times of the day?

Thank you so much for reading and replying to me. I feel so lost and cant seem to find any help from my h.v or anyone else, they just want to medicate which isn't stopping the vomit

P.s he burps right away after a feed but also seems to burp ages after eating with some of the spit up coming up with it, its like some of his wind just gets stuck. Not sure if thats related

Hi. My first was a refluxer. She was really bad, she used to reflux in her sleep and it would go into her airway and she would temporarily stop breathing. We were in hospital with her for it, we tried everything - carobel, ranitidine, Omeprazole, cranial osteopathy, naturopath, etc etc, every quack we could find got our money..... She was breastfed. The only thing that worked was me completely cutting out dairy. All the doctors said it wouldn't make a difference but my mum swore one of her 7 kids that stopping dairy was the only thing that helped .I had to do it for 4 weeks before I noticed a difference. She had goat's milk once weaned and then the milk ladder, she's now 3 and guzzles cows milk with no problem. We also started probiotics at a similar time as cutting out dairy and I hear there is some evidence for these so that might be worth a try too. I'll try and see can I find the brand we used because they had specific ones for breastfed and different for formula fed.

ProVen Probiotics for breastfed babies.
I found Oatly Barista Milk a good replacement. Never found a decent cheese replacement unfortunately.

Oatly is the best non dairy milk in tea. Almond is quite low fat and watery.
Vitalite dairy free spread is very good. And violife cheese - they do a nice cream cheese substitute. I don’t really like many of the other cheeses though, I find them a bit tasteless. I found yoghurt and cream /milk the key things to avoid.

I have found the Probiotics online so will order them and going to Sainsburys in the morning as I think they have a good free from range so have added that milk to my list along with the spread and cheese you have suggested @ElsieBobo

That's awful, it's so distressing. My dd was diagnosed with cmpa but the doctors did try omeprazole first and it just seemed to make it worse. My only useful advice is to keep going back to the doctors and asking questions, see a different doctor if you want a second opinion, we did and it was a real turning point. Also I take omeprazole as an adult for a stomach condition, be wary of other medicines, herbs, supplements etc as some can cause adverse reactions combined with omeprazole or just make it less effective.

If this dairy free diet doesn't work we will do as you said and go back and start pushing to get it sorted as he is getting worse.
