Missed Miscarriage - Surgical or Medical?

I’m hoping for some help. We unfortunately found out at our 10 week scan that we lost our baby at 6w2d. My body still hasn’t recognised it so I am booked in for a D&C on Wednesday. I have since been doing research on Ashermans Syndrome and have scared myself sick about not being able to conceive again… The risk looks to be 30.9% after a D&C yet it also says it’s a ‘very rare’ syndrome?
I have since researched medical management but the pain looks to be unbearable and I worry that I’ll need a D&C if that wasn’t successful. Online it says 80% of medical managements are successful and less invasive on future fertility.
We are desperate to have a baby - can anyone give me any advice on this and best option? I have a couple of days to decide…! But also desperate for this pregnancy to be over as I feel so sick 24/7 :( Thank you :)

I am sorry for your loss.
In your position I opted for a D&C, we went Private as the NHS unit was less than useless.
Apparently this can make you quite fertile and I got pg a week later

I'm just going to tell you my experience. Bit obviously this is just what happened to me.

I opted for medical management as the least invasive option. I bitterly regretted it. It was unsuccessful. Took all day. I had complications and I was losing so much blood and they could not stop it. So I ended up with an emergency D&C to stop the blood flow (so much so the on call consultant pushed me to theatre himself with a nurse rather than wait for porters. I lost over a litre of blood and had to have a transfusion.

It was utterly traumatic and left me very weak for weeks. I had mother missed miscarriage a few years later and opted for a D&C straight away.

I now have two beautiful boys.

You need to do what's right for you, but D&C is a very common procedure. I wish you well and I'm so sorry for your loss

Thank you! This is amazing to hear. We’ve been told I need to wait for a normal period before we try again. Was this different to what you were told privately?

Thank you so much. I’m sorry to hear you had such a traumatic experience. This was definitely one of my concerns which seems silly to then consider medical if it will result in a D&C anyway.
Do you mind me asking if you had any issues conceiving after your procedure or was it fairly straightforward? And did you try again straight away?
Congratulations on your two boys🥰

To be honest I don't think I was told anything. When I found out I was pg and expressed my shock to DH he said "oh yes, I think the Consultant said something about being very fertile"
I think they advise waiting for your period to return for dating of the pregnancy purposes but it isn't really an issue. I was worried and spoke to my GP, she said its fine. It WAS hard from a psychological point of view though if I am honest.
DD is 19 now so ti turned out Ok in the end

Kdubs1981 · 02/12/2024 17:59

Not at all. I don't mind

First missed miscarriages ending in emergency D&C I waited for my first period to pass and then got pregnant immediately (my 8 year old) . Second missed miscarriage a few years later. Same again, pregnant immediately after.

You are very fertile immediately after being pregnant (which you're body still thinks you are in a missed miscarriage)

There is no reason to think you won't get pregnant again and have a healthy baby. But I understand the fear that it will never happen. After my first miscarriage I worried about this. Sending good luck

I was in your position last week and im
so sorry for what you’re going through. I did not want medical management due to the pain/bleeding and lack of control over how long it would last/when it would be over and the chances of needing a surgical procedure afterwards. I wanted to have a D&C but had to wait 5 days for that. I just wanted the whole thing to be over so that I could start the process of moving on. They offered me a MVA for the next day. I was terrified about this as there are a lot of horror stories on here re MVA pain. I however am so glad I went for this. The pain was totally manageable. The local anaesthetic was the worst bit - felt similar to when you get a LA at the dentist but then everything numb after a few seconds. The procedure felt like period pain/pressure but was over quickly and I had gas&air ti help. My partner was able to be with me throughout it which also helped. Afterwards I had bleeding and mild period pain for approx 12 hours, spotting for a couple of days and then nothing. If your hospital offers that, please do consider it. Sending you lots of love

Hi @Bumblebee99 , I'm so sorry for your loss.

It might be that things happen 'naturally' before your booked procedure. This happened to me first time round.

Second time, I had a D&C under local anaesthetic. It was difficult but bearable. It was quick and I was supported in theatre by a lovely lady who held my hand and reassured me.

I was pregnant with my 2nd child shortly after.

Sending you lots of love and strength 💕.

Thank you for this, I’m so sorry for your loss. We were never given the option of an MVA but I have seen it come up a few times. I have a follow up ultrasound tomorrow to be told for the third time in 10 days the baby has no heartbeat (protocol apparently) so will ask if this is an option then. I am worried about having to be on my own without my husband for a D&C so I like the idea of an MVA for that reason!

Thank you so much for sharing your experience. I was hoping something would happen naturally as we’ve known for over a week now but it’s been over 4 weeks since the baby has died and I have had no pain or bleeding and just all the horrible pregnancy symptoms😓 Congratulations on your little one and I’m so sorry for your loss too💗

So sorry OP. My first pregnancy ended up with abnormalities resulting in a surgical abortion. It was the worst time of my life. I couldn't bare the thought of actually being awake and having to give birth. I wanted to be knocked out with no memory of it.
It took 18 months to get pregnant with that baby.
I was pregnant again the following month! Apparently the medication opens your cervix, which helps you conceive.

@Bumblebee99 the consultant who did the MVA explained that there are less risk of surgical complications with a MVA as they are just using a vacuum to remove the gestational sac but the D&C is more invasive. Good luck whatever you decide and happy for you to reach out if you have any questions about it ❤️

Hi I had im d&c in January past and this was complicated by retained products and I then needed a second d&c. Following this my periods returned but extremely light and I had three consecutive early miscarriages- I am being investigated for Ashermans. My most recent miscarriage I took medical management for it as i personally would never have a d&c again, I found the medical management very manageable but it’s seems different people have different experiences. Both have risk and complications unfortunately. The first miscarriage I took d&c as they could do it the next day because like you I just wanted the process over, but unfortunately for me the process constituted for 9 weeks after first d&c

julesagain · 02/12/2024 20:02

When I had a missed miscarriage a number of years ago, I opted for medical management as I have a huge fear of GA. With hindsight, it was completely the wrong decision for me. If it had happened again, I would have opted for an ERPC. Unfortunately, after this event, my health took a massive nose dive, and I never got my third baby.

legalseagull · 02/12/2024 20:04

I didn't wait for a normal period no. I bleed for a while after the procedure (like a period) and then got pregnant. The nurse at the clinic told me I would be at my most fertile so should go for it!

OneDayHope · 02/12/2024 21:48

So sorry you’re going through this, based on my experience my advice is stop googling (I did it too and it scared me to death) and go with the surgery. I say this because I’ve had 2 mc, both I initially had medical management and both times it failed miserably, this meant the physical side of the mc was prolonged and prevented me from processing the mental side of it all, it distressed me even more both times. My experience of surgery both times was great, well looked after, well informed, barely any bleeding after and cycles back pretty sharpish.
Take care and I wish you all the best x

LBOCS2 · 02/12/2024 22:01

I had one medically managed miscarriage and one surgical. I found that the surgical gave me much better closure and was easier to recover from as it was done and dusted very quickly, unlike the medical one.

I also got pregnant again immediately afterwards without having a period. She's just turned 12.

Flatandhappy · 02/12/2024 22:09

I had two D&Cs (first pregnancy miscarriage at 12 weeks, third pregnancy it was a missed seen at 12 week scan). I got pregnant again quickly after both. Good luck.

Pyjamatimenow · 02/12/2024 22:30

I had surgical at 12 weeks. It was very straightforward and I had very little bleeding afterwards and no pain. Got pregnant with dd next cycle

Bumblebee99 · 04/12/2024 18:05

Thank you for all of your lovely and supportive messages! I went ahead with the D&C today and as far as I know all went smoothly. Feel okay right now with little pain or bleeding. Fingers crossed for a healthy and sticky baby in the new year ❤️

Hi there I know this is a slightly older post but I am due to get an MVA on Wednesday, would you be willing to share how long it took you to conceive after your procedure? 🌈 thanks xx
