Me again and my vagina 😭

I've posted several times over the previous month's about severe itching and soreness around and inside and also anus.
Ive already had an internal exam by a doctor two weeks ago she said all looked fine , although she prescribed Clobaderm ointment incase!
I still think it could be lichen sceloris, she's referred me to a dermatologist but who knows how long the wait will be.
Over the last 24 Hours it's ramped up I can now see what look like sores just inside , red raw, weeing is like a million paper cuts and I'm holding on to the sink for dear life ! I've an appointment with 111 face to face doctor at 5pm.
Here is what I've tried so far.
Clobaderm ointment (very reluctant to use this though without a LS diagnosis)
Vagisil cream
Vagisil powder
Emoillent for washing
Ovestin which has helped with urge incontinence.
I started in HRT 7 weeks ago I use Everol Sequi 50 patches and then the conti for 2 weeks , I feel no different but determined to see if through for 12 weeks, I had the itching before this but it's definitely got worse.
I wash my underwear separately, I've purchased new underwear , I don't use fabric conditioner and little powder , I dry it outside where possible.
I've tried thrush treatments and fluconzole tablets , I've had an antibiotic course for a UTI but I don't have a UTI.
My GP didn't take any swabs !!
My diet hasn't changed , I drink lots of water. I had a Hba1C blood test done in Friday result will be in tomorrow in case it's type 2 diabetes.
Sex is of the question, exercise is killing me , I have a physical job and don't know how I'll cope this week.
Who knew a vagina could cause so many issues.
Is there anything ive missed wise ladies ?
I'm sorry for the long post but I'm feeling pretty desperate today.

Give the clobaderm a proper go, use it a couple of times a day for a week. If it is lichen sclerosis or similar/auto immune, the difference will be huge in a couple of days. But you have to use it properly, light irregular application wont make a difference. You wont do harm in a week, and if it works you have your answer.

Thank you @LightlySearedontheRealityGrill I am prepared to do that but I'm worried it's something else with these new Lumps/lesions , I've put nothing on today so that the 5pm doctor can see it all in its glory , I'm dreading being examined as the pain is excruciating right now.

I have lichen sclerosis, it is quite obvious as it leaves whiter almost depigmented areas where you have had an attack. It has been quite manageable with the steroid cream so far. But pre treatment it was unbearably itchy, and I think if you are scratching it you could easily cause more damage and even infection if you break the skin.

Thanks @LightlySearedontheRealityGrill I'd like to know either way if it is lichen sceloris,I've done so much reading about this and various other conditions. It's affecting every second of my life and it's unbearable today. I don't know if the get patches are making it worse ?? Trying to get an appointment is a full time job , I got lucky today on 111 I guess.

I suffered with this for years and years and I seem to be in remission since I started low carbing .
I can't explain it.

@memyselfi I'm reducing sugar and carbs so hopefully that's a good thing. I'm hoping for my Hba1C result tomorrow morning so I can rule that out or in !

The Royal Free hospital has a specific vulval medicine department. Is there anything like that near you OP? Or perhaps you could do some googling and find a private specialist? I have got a friend who is going through this right now and she has see 4 different GPs and got absolutely nowhere. She is going to pay for a private consultation with one of the RF doctors who does some private practice. I can only imagine how painful and distressing it must be. (RF is her local hospital).

Use dermol 500 to wash with daily. Don't wipe with toilet paper alone after a wee. Use dermol on the loo paper

I also find a daily anti histamine helps and also eumovate cream manages it effectively

Steroid cream works for me but I have to keep it up or it comes back quick. Problem is I just don't bother when it's not itching!

Oh OP I really sympathise - I had terrible vaginal atrophy because of menopause which is finally under control through a combination of

vagifem - loading dose every day for 2 weeks then 2/3 times a week
ovestin externally twice a week combined with yes VM moisturiser which i also use internally twice a week

nothing else has worked

Thanks all for your replies ,.I just saw a GP (out of hours clinic ) she had a look and said there are several skin tears so that's what's causing so much pain , she suggested I use the clobaderm I've been prescribed which I will.
My Hba1C results were back and were 40 so no diabetes or pre diabetes now my previous result was 44 so I'm pleased about this.
She suggested also using replens which I've not used before.
She thinks it could be dermatitis type thing , but I have a referral for this however long that might take.
Thanks for all your advice most things I'm already doing but will keep up with them and try and grin and bear it , I literally break down in tears a lot about the sheer pain from this.

Also forgot to add she tested my urine and there is glucose in it !! But my Hba1C is clear , she had no answer for that !

Have they done full blood tests? Are your kidneys ok? If so, this is going to sound drastic but as someone mentioned above, a low carb diet can help autoimmune diseases and inflammatory conditions like dermatitis which may also be autoimmune. It's nothing to do with your blood sugar or diabetes, it's about leaky gut and things like grains and oxalates. If you really are desperate you could try a carnivore diet, even just for a week, and see if it helps. At minimum it functions like a kind of exclusion diet by removing all the common dietary triggers.

@Pollyputhekettleon yep I've had full bloods as far as I can read , hba1c , vitamin d , thyroid all okay.
I've started 16:8 but will try and ensure during the 8 I don't touch carbs , it's a difficult one as I have a physical job but only I can help myself , thank you for your kind advice

Can I suggest that, instead of Replens, you try YES VM

Similar here. Only thing I'd add to the underwear thing is I was told to wash them in just water. So I take them into the shower with me and give them a good rinse in water then hang out to dry. Has helped a bit.

@33goingon64 yes I do that , I use the washing machine still but add nothing to it at all wash in a high temp and then an extra rinse , what a waste of energy but needs must !

All the symptoms you are describing including a UTI that wasn’t a UTI can easily be solved with oestrogen cream.

@RetiredEarly I've been using Ovestin estrogen cream for the last 7 weeks since I started Everol Sequi , I did the first two weeks loading dose and then I use every 3/4 nights. At first it helped now the itching , paper cuts and soreness has gotten so extreme I can barely put anything there hence all my GP appointment regarding this.
I've had the itching for a long long time but it's definitely got worse since starting HRT but I'm determined to see the 12 weeks of Everol Sequi out , so far at week 7 I feel no different and have 13 symptoms which are still bothering me , but the itch and the cuts and soreness is definitely the number one issue right now !

@RetiredEarly yes perimenopausal which is why I went to my GP initially 9 weeks ago and she prescribed the Everol Sequi and the ovestin which I was happy to try
