How many of your eggs got to blastocyst? And how many euploid/ normal from PGTA testing?

I've seen various drop off rates on the internet and am interested to know outcomes from real people.

My AFC is 20, my AMH is 12.5 and I am over 40. The clinic have suggested three rounds of short protocol maximim stimulation to hit my ovaries hard (embryo banking) + PGTA testing. I have only tried naturally for a few months and am considering skipping straight to this for time reasons. No male factor issues.

Is the below what I might hope for per round?

16 collected?
10 fertilise?
5 make it to blastocyst?
Maybe 1 euploid (I realise zero is a possibility)?

Am I getting the drop off rate at any stage wrong?

For those who are more seasoned than me:

  1. How many eggs were collected relative to your AFC?
  2. What percentage fertilised?
  3. What percentage made it to blastocyst?
  4. If you did PGTA testing, what percentage were euploid?
  5. How old were/ are you?

13 collected, 11 fertilised, 0 blast
11 collected, 9 fertilised, 4 blast, 0 euploid (pregnant from a mosaic one though)
5 collected, 5 fertilised, 4 blast, 1 euploid (miscarried)

same protocol for all. I was 40 at the time. It’s a numbers game that often makes no sense.

sirensong · 07/07/2024 13:56

@contentsmayb thank you for your reply. What protocol were you on btw? I'm new to reading about this so don't fully understand who gets put on what. The consultant insisted short protocol wasn't a fob off version because of my age but online seems to say it's for suspected poor responders.

Always28 · 07/07/2024 14:44

I was 41 for my first collection and just turned 42 for my second

Round 1
4 eggs collected
4 fertilised
3 made it to blastocyst
0 euploid

Round 2
9 eggs collected
9 fertilised
9 made it to blastocyst
4 euploid

Not much changed between rounds - it was just better luck the second time around.

sirensong · 07/07/2024 14:47

@Always28 amazing results second time round. Have you had transfers from your 4?

OP posts:
Always28 · 07/07/2024 14:49

We had a failed transfer in January. I’m now 10dpt for our second go. Did a pregnancy test yesterday that was positive, so feeling hopeful for this one 🤞🏻🙏. I will keep you posted!

I honestly can’t remember what my AFC was. I also think you just can’t really predict what will happen and just have to wait and see. But the plan they have made for you sounds positive.

sirensong · 07/07/2024 14:59

@Always28 congratulations! Everything crossed for you.

short protocol. I too don’t fully understand. They will do your first cycle as a trial and then adjust things based on what’s needed. I responded well all 3 times. Third time I actually had 16 follicles, but somehow only collected 5…

Clocloxx · 07/07/2024 22:40

Age 30
AFC 15
AMH 3
Eggs collected 4. 0 fertilized

Age 31
AFC 11
eggs collected 2. 2 fertilized. 0 blasts

These were both on high stims
Will be doing a 3rd ivf with mild ivf soon

sirensong · 07/07/2024 23:22

@Clocloxx did they offer any explanation for your unlucky response given your decent AFC and young age? Very best of luck next time round.

15 collected
1 blastocyst that was confirmed PGTA normal

And now a healthy baby 🙏🙏🙏

SO incredibly grateful for IVF

ricefields · 07/07/2024 23:39

To answer your qus…hope it helps

How many eggs were collected relative to your AFC?
14 eggs first round. 15 eggs second round
AFC was estimated to be similar, maybe a bit less

What percentage fertilised?
11 the first round. 12 the second round

What percentage made it to blastocyst?
2 blastocysts in the first round. One miscarriage and one 6 day 5CC which we froze

1 blastocyst in the second round, 5 day 4BB which tested normal. We tested even though only one embryo as couldnt cope with going through miscarriage again.

If you did PGTA testing, what percentage were euploid?
1 was euploid, we got lucky

How old were/ are you?
37 on first round. 38 on second round

sirensong · 07/07/2024 23:41

@ricefields amazing! Congrats.

Thank you. Wishing you so much luck.

SH998 · 08/07/2024 04:18

Was 38 at first collection, 39 at second collection and baby boy was born at 40.

First collection below:
14 collected
8 fertilised
2 made 5 day blastocyst
1 miscarriage
1 failed

Second collection below:
8 collected
8 fertilised
5 made day 5 blastocyst
Double transfer failed
4th transfer gave me my little boy
2 embryos still frozen

We didn’t do any testing so know how extremely lucky we were to have a successful pregnancy.

Good luck x

sirensong · 08/07/2024 07:37

@SH998 your body may have effectively done the testing/sifting instead, though of course there are many reasons for miscarriage or failed implantation.

A wonderful result to end up with your little boy - congratulations!

SH998 · 08/07/2024 09:04

@sirensong thank you, I know how lucky we were to get a positive result.
Good luck to you x

ricefields · 08/07/2024 10:22

Congratulations @SH998

It’s interesting to see on here anecdotally it’s often the case that the later round when you’re older gives you the baby despite being older…which makes me think that up to a point it’s really a numbers game. I think I read somewhere that 75% will succeed if they have 9 collections, obviously money is a barrier to this! And also emotional and mental health.

I used to follow Tim and Celeste on YouTube and their story was so motivating for me to keep going. They went through so much and over ten years of infertility but got there in the end with a beautiful little girl.

Good luck to everyone on this thread.

DaisyTee · 08/07/2024 13:07

It's really interesting to see everyone's stats, congrats to those who have been successful and wishing lots of luck to those still going.

My AMH is 11.8. I had 2 cycles at 41 and I'm currently on my third ICSI cycle having just turned 42. Issues are age and low sperm morphology.

Cycle one
8 eggs collected
7 fertilised
2 blasts
x1 double transfer - BFN

Cycle two
11 eggs collected
7 fertilised
4 blasts
x2 double transfers - x2 BFN

We decided against PGTA testing so far in case we don't get many blasts and want to give each a chance.

Very best of luck with your cycles, all fingers crossed for you x

Faith2024 · 08/07/2024 14:08

22 eggs (over 3 cycles).
16 fertilised and 6 blastocysts. 2 genetically normal. I was 41-42 so around the given stats of 33% of embryos will make it to blastocyst stage, and of those 33% are likely to be euploid. Due to my age it was important for us to get the testing. We had clear cut results back from our PTG-A so I am glad we did it.

I also did embryo batching and I was on max dose of stims.

Good luck!

sirensong · 08/07/2024 15:17

@Faith2024 thank you for your reply. How did you get on with your two euploids? And can you remember your starting AFC and AMH?

OP posts:
HopefulllHolly · 08/07/2024 15:23

Age 37, first cycle. AFC 6 or 7, I can’t remember! AMH 3.9

3 eggs collected
2 mature, both fertilised
1 day 5 blast (untested because there was no point when we only had one! The other arrested day 3)
Fresh transfer and now almost 12 weeks pregnant.
VERY LUCKY 🍀

sirensong · 08/07/2024 15:26

@HopefulllHolly BOOM!! This is really really great.

OP posts:
Faith2024 · 08/07/2024 20:21

@sirensong our first transfer is later this month.

AFC is just a guide, mine was 5.8 and one cycle we had 5 eggs and another 11 eggs, both same protocol. I have come to believe that ultimately it either happens or not. Good care helps but it seems like it’s all chance.

CharlotteYorkMacDougal · 09/07/2024 09:07

I was 38 for all three rounds and my antral follicle count was about 30 (I have PCOS).

Round 1 (standard IVF)
21 collected
18 mature
11 fertilised
4 blastocysts
1 euploid (subsequent transfer & live birth)

Round 2 (IVF with ICSI)
16 collected
8 mature

8 fertilised
5 blastocysts
1 euploid (still frozen)

Round 3 (IVF with ICSI)
18 collected
15 mature
12 fertilised
6 blastocysts
3 euploids (still frozen)

I agree that much of variation in the results between cycles is down to chance and not something you or your treatment team can influence too much Sad Though I do personally think that your choice of clinic is important.

When I was researching it was very difficult to find information about what numbers could be ‘expected’ at each stage but the Vienna Consensus by ESHRE gives key performance indicators for embryology labs which I found very helpful.

Wishing you the best of luck with your treatment!

Faith2024 · 09/07/2024 10:18

@CharlotteYorkMacDougal I agree with clinic choice too, most fertility consultants have good competency but things like lab capabilities and embryologists can be what makes that difference.
