Extremely high progesterone on embryo transfer day

Hi all
I had a double embryo transfer yesterday and asked the clinic for a progesterone test for reassurance in case of too low levels. I have been taking progesterone pessaries 3 times a day since egg collection on the 23rd August 2023. I didn't expect result to come back 3 times the clinic's top normal range so 225nmol/l. Plenty of scientific articles I googled say that's detrimental to endometrial receptivity, implantation etc. The doctor yesterday said it was high but didn't recommend coming off the pessaries.
However, I'm really worried we've wasted 2 of the 3 embryos that we got over the last year of 4 attempts at egg collection.
Any advice and experience would be much appreciated! Xx

@Summerishere83 aaarrgggh did a typo that muddled what I meant...I meant your level at transfer will be very different to your level at trigger and the studies are about levels at TRIGGER! High levels at transfer are fine x

Hi Ladies Anyone can suggest after first round of ivf failure when they do the nest transfer
Also my follow up appointment with doctor to discuss this on 18 sep
Ease help me what should i ask doctor

@Preet89 what type of cycle have you had, collection with transfer or FET? What were the results of the treatment you've had so far?

@Summerishere83 Mumsnet is a great place for support. I've come to the end of my IVF journey now with a DD4.5 and a DS 15 weeks old but am always happy to lend support. Best of luck xxx

I had my egg retrieval done on Aug 16.., 14 eggs retrieved, 7 mature and 4 fertilized
One fresh transferred on 21st Aug and on day 8 I started to bleed
Still got 2 frozen

I did a test this morning and it showed a faint line 🥹 my husband also can see it so I'm not imagining 🤓
I'm very cautious what to think, so many more hurdles to jump and milestones to reach!! I've had a chemical and an early MC so far from thinking I'm pregnant yet, just hoping it will be darker tomorrow morning 🤞🤞🤞
Thank you all for putting my mind at ease about the progesterone ❤️ I've had some cramping in the morning so I decided to up the progesterone and added another pessary to the 3 I'm already taking per day, hope that's a good idea, it seems there's no such thing as too much progesterone 🤞🤞🤞

@Summerishere83 you're 5dp5dt is that right? This is good news, the line will be faint at this point.

@Preet89 I started bleeding like this in my fresh transfer too and expect my progesterone was too low. It took me a long time to become pregnant with IVF in a fresh transfer as the embryos aren't tested the likelihood of success is 1 in 3. You should be able to have an appointment with your clinic to discuss a frozen transfer, it's good that you have a couple frozen to use still.

It's good you have 2 frozen embryos! I suppose the next thing to do will be a frozen transfer and that's supposed to have higher success rates than fresh transfers.
I have to say, based on all comments in this thread, insisting on having sufficient progesterone is a must!!

@Summerishere83 did you say your transfer worked with your 225 progesterone? I’m worried as mine is 202 and I’ve got transfer tomorrow. They’ve asked me to reduce the pessaries to 400mg twice a day rather than 3 times a day and I’m on PIO every third day. I’m stressing my lining will be completely out of sync and I pay a lot of attention to dr victory as I have adeno but he said his limit is 150!

@Gardenlady543 you said levels don’t always matter it’s more the time it’s started? I did a PIO at 9pm last night and 400mg of pessaries this morning at 7am and my blood test was at 9am today.

worried I’m gonna fail before I’ve already started!

@Loz2467 I was worried my levels would be too high as well and was reassured by someone that the expensive clinic in London, people often have a result in the hundreds.

@Gardenlady543 thank you :) this isn’t the type of stress I want the night before transfer and working till 10:15pm. I’ve had someone tell me it doesn’t matter but then I’ve seen studies online saying it can be too high and the endometrium not receptive or in sync. Seems the OP had hers work but it looks like it was a fresh transfer. I’ve spoken to someone on IG and hers was 180 and was told to drop a PIO dose and she was at 115 the next day which was her transfer day however hasn’t done beta yet.

My progesterone was 150 on transfer day and 180 two weeks later on my OTD. The clinic was happy with the numbers and told me to continue with the medications. It was a frozen transfer.

So far so good, the transfer worked and I'm pregnant but I haven't had a scan yet.

@Loz2467 hi there, I'm very happy to report my transfer was successful and I've got 10 month old twins now ☺️

the transfer was with one fresh embryo and one frozen so hopefully that puts your mind at ease a little 🤞

however, I do remember how I was driving myself crazy with researching the hundreds of questions in my head. I remember my doctor wasn't worried at all but not dismissive of my concerns and said that the reason they don't routinely measure progesterone before a transfer is because it doesn't matter, the pessaries should provide enough and too much isn't an issue, something along these lines...

Hopefully you'll manage to rest tonight and be cautiously excited about tomorrow, high progesterone shouldn't be a problem, on the contrary 🤞🤞🤞 hope it all goes well ❤️

@Loz2467 personally I would feel more comfortably staying on the dose and telling the doctor I don't feel comfortable dropping down, but equally you need to make sure you and the doctor agree on what the plan will be.

My feeling with the studies, is that the patients aren't likely to be the complicated patients, with reproductive immunology patients they always want it higher.

My clinic only wanted my progesterone above 100, they made it sound like the higher the better

@GreenLemonade gentle congrats on your transfer being a success and everything crossed for your scan. That’s reassuring you had higher prog levels and it worked.

@Summerishere83 that’s amazing you have 10 month old twins! Congrats! Yeah I was spiralling but I’m starting to feel better now. I’ve thrown a lot at this transfer so just have to see how it goes now.

@Gardenlady543 yeah I am worried to drop to be honest because my test was literally 2 hours after my dose and like I said my PIO was last night at 9pm. I bet if it was a day after my PIO it would be lower? I don’t know.
i think it’s the PIO that’s made my levels shoot up and yeah my clinic aren’t hot on immunology.

I had 4 transfers and the only successful one was when I had high progesterone.
