No. Unpredentedly pressured situation, breakdown in communication between people at the sharp end, tragic outcome. Desperately sad and, yes, avoidable but what use is sacking the man at the top whose record is otherwise excellent?
I hate the media/political response that says a matter is septic til someone's been singled out and sacked then suddenly it's treated as being settled. Far better to focus on sorting out what went wrong and how it can be prevented - long term and systematic action isn't as satisfying for onlookers but it's what's needed.
Sorry - bit of a rant but so much of the comment and speculation is incredibly short-sighted.
