Medicine - heading into 5th year!

DD is heading into 5th year and starting to analyse the 2026 application ratios. Anyone else starting a spreadsheet? @mumsneedwine - you're normally my guru! London is back up to 2.3, apparently...

London will always be popular for some reason (I say this as a Londoner born and bred). Afraid I don't do anything after application to Uni as I'm a teacher ! But the new (stupid) algorithm is not great and I know there is talk of changing it back to some kind if academic ranking. The UKLA should make this easier and fairer than before.
Majority get their first choice though.

Wishing your DD the best of luck @KingscoteStaff. DD is now also in 5th year but still has another year to go as hers is a 6 year course. I’d like her to be in London but I think she may tick Thames Valley as ‘home’ territory.

Nice to hear from you @HoneyMobster ! Thames Valley is 1.29 this year, so fingers crossed.

I think DD would like to stay up North if a group of her friends also stay and they can live together. The North Hospitals are all quite central, so even if one gets North Tyneside and one gets Darlington, they could all live in, say, Durham, and have a 20 min drive to their hospitals.

Personally, I'd rather have her home for F1 so I know there's food in the fridge for her, but even if she did get London, she might end up in Croydon or Redbridge, so having free accommodation in Fulham wouldn't be any help!

They will end up where they end up I'm afraid. And will keep on moving regularly for the next 9 years (unless BMA sort it out). DD did her foundation 4 hours away - had an amazing time, made new friends and doesn't regret it for a minute. Now in a nearer deanery but moves every 6-12 months to a new hospital (chose this deanery as it's quite small so everywhere only an hour way from centre).

All they can do is apply and hope the algorithm is kind (DD was the last year they used academic ranking and SJT so kind of knew where she'd get).

Many F1s are new to an area and they run kind of freshers weeks and activities for them to get to know the others.

@HoneyMobster system might change 🤞. It does seem v silly that there is no benefit at the moment to working hard and getting a higher academic score. A random algorithm seems v lazy (but hey, it's the NHS management so not sure why I expect anything else).
Apply in order of where you want to live and keep 🤞🤞. Most do get 1st choice - it's the ones that don't and end up with 18th choice that are being stuffed. Doctors really are not treated well in this country 😡

DS is in this cohort too and has been not very patiently explaining to me how the allocation works. Namely, if your first choice deanery has a first pref ratio of above one, there is no point putting a second choice also above one as it's mathematically impossible that you'll get it.
It's all so arbitrary anyway. You can choose North West, planning to live in Manchester and find yourself living on the Isle of Man for 2 years (as ds's friend did). It's a crazy system. Good luck to all your young people for the next phase.

@Haffdonga new to this (DD going into 4th year) and your post has made me realise there is some research to do!! We are close to Manchester and whilst its early days for DD to decide where she wants to be, I had thought North West would be good and she'd have the option of living at home. Isle of Man😮?!!!

@Africa2go unfortunately you choose your deanery and then choose the track (the order of the 6 foundation placements). And the order they are given out uses that same stupid algorithm again. So if have a lower number you might not get your first choices of jobs. Some deaneries are bigger than others and spilt into smaller ones, but hospitals can be hours apart.

The advice my DD always gives (and was passed to her) is to make sure your 2nd placement of F2 is the 'easiest' one (eg GP, Psyc, anything supernumerary where you won't be in call or many nights). As this is when you'll be applying for speciality training and maybe sitting your MSRA exam.

@Haffdonga There is a really helpful flowchart on the Foundation website - I consult it when DD is running short on patience! For anyone thinking about London, they have put up the PowerPoint slides they used to explain the process last year, which I found helpful.

The only thing I can’t find at the moment is the data on how many of the London F1s applied for each area, and how many got their first choice.

It's a gamble. If you want one of the five deaneries which include London, Oxford, Bristol, Birmingham or Manchester, then you might not get your first choice. In fact, in the case of London, you are more likely to not get your first choice and some years the Oxford and Bristol deaneries are not far behind.

My dd applied in the second year of the random number allocation system. She really wanted London as that is her home city but decided it was too much of a risk, so went for one of the other oversubscribed deaneries (where she had been at medical school). She got her first choice but the friends that she had intended to share a house with didn't.

As pps have said, even if you do get the deanery that you want, the same process then applies for the locations/jobs within that deanery. Scotland and Wales are both single deaneries so the distance between where you want to be and where you end up can be huge. And there are the anomalies of the Isle of Man, the Isle of Wight, Jersey etc within all this which she was keen to avoid.

When it comes to choosing the actual jobs in the second phase, she didn't get the job that she wanted and was initially disappointed. But she has loved the placements that she thought she wouldn't like and not been quite as keen on the one that she thought she would love. She has also managed to swap her second year with another doctor for something more in line with what she originally wanted.

Overall, all you can do is cross your fingers and hope for the best!

Following. Ds just about to start y5 at Newcastle. Went via partners route so I believe guaranteed North East but very torn if he wants to stat or fo elsewhere

I’m not sure that the Partners Scheme guarantees an F1 placement in the North - I think they just apply for a Foundation School like any other graduate.

Having said that, the first choice preference ratio for North is less than 1, so if they put it first, they should be fine!
