Long term itchy rash, photo inc. GP no idea

Dh has had this weird rash thing for over a year. All over his body from legs to scalp. Extremely itchy. Starts off as small bumps which have clear fluid and then turns into larger welts.

Gp did prescribe loads of steroid cream, powder and wash which seemed to help a bit but not cure. And as soon as he’s run out it’s flared back up intensely. He did have relief from the itching while using the cream.

Nobody else affected, we haven’t changed wash powder. Normally fit and well.

I did wonder coeliac as dd has it and I know it can affect the skin. He’s waiting for blood test results. Any other ideas ?

Are they thinking pustular psoriasis or pemphigus. How frustrating it's not been diagnosed, what have they done so far, surely there must be a doctor in the hospital who has dermatology or rheumatology experience

At least now it’s being taken seriously - the GP sounds pretty hopeless - it’s a worry really when they can’t recognise potentially life threatening illnesses. Let us know how he gets on if you feel able.

OMG OP this is shocking. I really hope your DH is diagnosed and treated soon. I know your DH is not the position to do so now, but he really should make a complaint about that GP when he is sorted.

Is there a dermatology ward in the hospital or wound care nurse, should the blisters be covered, is he on decent painkillers.

I had what looks very similar and was eventually diagnosed with toxic pustular derma. I was hospitalised for 2 weeks IV antibiotics and daily dressing changes. My liver function was also effected for a while.This was about 10 years ago,they never found out what caused it and I've never had any further issues. I hope your husband gets some answers and helpful treatment very soon.

I'm surprised he's not on IV antibiotics to prevent infection. Perhaps tomorrow the dermatology consultant will authorise this

It looks a bit like a viral rash- severe shingles or enterovirus/coxsackievirus type but I’m deffo no expert. Really hope he’s much better soon.

He’s not on any pain killers. They’ve prescribed steroid cream but can’t give it as they said pharmacy is closed at weekends. 🤷‍♀️. So if they don’t have a certain medication on the ward he can’t have it. I know for a fact there’s an on call pharmacist, they just haven’t rung them!

The wound care nurse doesn’t work weekends. Some of the orange blisters are now the size of a 50p. Some have burst and are constantly weeping and they’ve put an absorbent pad on those.

if it is that generalised pustular psoriasis then stuff online says it’s an emergency, treatment needs to start straight away, he should be treated like a burns victim and on an IV drip. I’ve told him to keep drinking plenty.

apparantly one doctor yesterday asked another doctor about a drip and the second dr said dh doesn’t need one. His obs are ok still today so that’s reassuring.

Ask for a telederm review. Gps have ipads with super zoom cameras, pics go directly to derm and they advise treatment within 2wks. Was fab when I worked in gp many moons ago

It sounds dire, there are certain medications that are essential, I can't believe he has no pain killers prescribed, a surgical ward or itu outreach could advise on dressings, bloods, have you spoken to his nurse and asked why he hasn't been seen today.

As much as I believe he clearly needs to be in hospital it seems very odd to have kept him in and then say they can't treat him and not to provide any IV antibiotics.

🤞 that someone sees him tomorrow and starts a treatment plan and start to seek some answers.

Start being assertive today. I'm hoping the morning round will bring in some results.

I can't believe the pharmacy is closed all weekend that's appalling.

You'll need to speak to PALS we photographed sons medical records, they weren't happy. They had noted every time DIL had approached them to ask for meds, attention for her husband. But it got him morphine and an IV drip with penicillin and fluids.

There's always an oncall pharmacy at evenings and weekends, some open on Saturday morning, the nurses can also phone round other wards or check if there is any stock elsewhere on the computer. Leaving patients without medication is really bad. Worse comes to the worse you get a green prescription for a chemist. Not having painkillers is just cruel.

I was once in a hospital listening to a doctor tell the pregnant patient in the bed next to mine that she needed specialist medication to dissolve the blood clot in her lung (covid) but it needed to wait till Monday. I was appalled I really think getting necessary medication should be a 24/7 thing.

Where I worked there was a list of essential medications that could never be missed, if they were it was a recordable drug error that got investigated

The GP really has failed him letting it go on for a year and not sending your dh to a dermatology specialist.
