Doctors not agreeing on cause of rash (photo uploaded)

I'm at my wits end with this rash on DS's foot. He's autistic and non verbal so not able to tell me if it hurts, he doesn't appear to be in discomfort with it but it is obviously itchy.

This has been going on since last summer, we were first told it was ringworm, then psoriasis, then eczema and nobody seems to agree what it actually is.

The larger "ring" started small and has gradually gotten bigger and the smaller ones appeared about two weeks ago. He did have a small amount on his other foot which has now disappeared.

We have another appointment tomorrow and I'm going to ask for a referral to dermatology but just wondering if anyone has any idea what it looks like? Thanks

What creams did they given you to try and did any have any effect?

Vasculitis? There's lots of types including revivalresearch.org/blogs/urticarial-vasculitis-symptoms-and-treatments/ urticaria. Could you ask for a rheum referral? And autoimmune bloods?

Thanks @LoreleiG He's had Hydrocortisone which helped somewhat but then seemed to stop working. A cream called Terbinafine which didn't do a thing and another anti fungal which I can't remember the name of and that didn't work either. Just at a loss!

@Bennyontheloose Thanks, will look into that and ask for bloods and a referral at the appointment. I think everything needs ruling out at this point.

Looks like either ringworm or pityriasis rosea to me, although pityriasis usually happens on the torso. Both are itchy.

pityriasis rosea, they did initially diagnose ringworm and that's what I thought it was too but nobody seems to be able to agree.

My daughter had a weird rash once and I’d definitely recommend a dermatology referral as what hers was (which wasn’t serious) wasn’t something the GP would ever have suggested.

@LoreleiG eczema was considered at one point so it could well be that. Thanks for the link, will have a look into it. I'm definitely going to push for a referral, just not getting anywhere with the GP.

Looks similar to what I had which was diagnosed as psoriasis brought on by strep.

Had it for months. It was insanely itchy. No creams worked and it just disappeared in the end.

Really made me think of Pityriasis rosea as this starts with a ‘herald patch’ I think it’s called - the one large spot you refer to - and then lots of other spots grow around it. But as PP said, it’s normally on the torso. DD had it all over her chest and stomach.

Just wonder if he's taking an anti inflammatory pain killer cause I had something like this that appeared in roundish red bits that joined together? It went away when I stopped taking it

@GoldenMeadow psoriasis was mentioned by one doctor but they seem to think it would be more on the soles of the feet rather than the too but not impossible I suppose. Glad to hear yours healed!

@RulerofCatsjustkidding no no ant inflammatory. He's had ibuprofen occassionly when unwell but I wouldn't say often enough to cause a rash..

@ohxmastreeohxmastree I did look that up at pps suggestion and it does seem that it almost always starts on the torso and then spreads, so doesn't really fit with DS's rash. It does look very similar though. Poor DD, hope she's well now.

Looks like what l get often..( Urticaria) has he had asprin/ Ibuprofen?
Sea food/ been in nettles?
I'm allergic to Asprin and Ibuprofen and they gave me Urticaria.. obviously l don't take them.. but l took two Ibuprofen at my daughters last month ( mistook for paracetamol) and that's the rash l got.
Took some antihistamines.

Thanks @Moier that's interesting. He only has ibuprofen very occasionally when unwell and none of the other things you mentioned. He's had it for over 6 months and it's gotten gradually worse. Hopefully will have some answers soon.

I’d have said annular psoriasis but not sure if there’s enough central clearing. Worth another chat with the GP again especially if steroids haven’t helped.

Thanks @TheWrongAllmanBrother, just googled annular psoriasis and I agree the centre doesn't look clear enough but it could be a possibility. Got another appointment tomorrow and going to push for a referral.

It looks like my psoriasis. I have it on my shin. The doctors are always baffled as the text book says it should be in certain places yet mine (and my dad's) are everywhere when we have outbreaks.

Thanks @Pieceofpurplesky, it could well be psoriasis. I jump from thinking it's one thing to another. That's what threw me when they said it wouldn't typically be on the top of the foot but as you say yours develops in places that aren't typical. Have you found anything that's worked?

Have you tried antifungal creams? I'd say ringworm too.

Does it go a bit white and flaky if scratched lightly with a fingernail? If so it could be psoriasis.

@BurbageBrook yes sometimes the centre goes white and flaky but unsure if that's after he's scratched it.

Well in that case I think it might possibly be psoriasis. Have you had a referral to a dermatologist?
