HELP REQUIRED FROM DOCTORS OR MEDICAL RECEPTION RE PATIENT MEDICAL RECORDS

Hello everyone,
Firstly thank you so much for reading my thread, its truly appreciated. Unfortunately I believe that after raising a complaint with my Doctors in regards to discriminating against my Mental Health, this has lead them to make changes to my existing patient records. I just want to get some clarity from someone who understands the system that is used by nhs Doctors and reception.
Firstly I'd like to ask if someone could confirm what the system is called?
The query that I have is that I believe that a medical receptionist has updated my records an this has been changed to make it looks as though the Doctor was trying to get in touch with me which wasn't the case on the date the update was made. Is there a way that I can ask or see when this entry was made and updated including times and dates?
I'd be grateful for any information anyone is able to provide.

My NHS app shows me this sort of info. Have you checked on yours?

Hi, midwife here. Lots of different services, including GP's use different systems, this is why it's a ballache to get joined up care in the NHS some of the time!

All good healthcare systems, to protect GDPR will have an audit trail of who, when, and what was changed if amendments have been made.

I would maybe contact the Practice Manager?

Hi there, thank you for this. I have just updated my details on my nhs app however I believe the changes would show on the internal system rather than the records accessible by patients.

Ah I see that makes it difficult I can imagine.
Yes the Practice Manger has been overseeing my complaint so sadly is part of this. They're trying to say that a doctor called me an hour and a half earlier than my appointment was scheduled but actually that call was the receptionist contacting me to say the Doctor was refusing to see me because I couldn't make it in to the practice because of my Mental Health that day. I just find it sad they'd rather hide it than just apologise and it worries me for other patients and potential hidden negligence.

As part of GDPR you should be able to make what's called a subject access request. This means the practice will hand over any and all records they have that pertain to you. You have a right to see what data they hold on you.

I'm not sure if you would have the right to get the information changed -- if it's their word against yours that the doctor tried to call you that might be hard to prove, but you should be able to see what's been written in there.

Google 'subject access request' for more details - official information will come from the Information Commissioners Office (ICO). Hope you're ok.
