“appears distressed with restricted range and reactivity”

and

“has only fair insight into current issues”

Restricted range and reactivity would mean that her apparent emotional reactions to everything are within a fairly narrow band and she doesn't always emotionally react as strongly as you'd expect in normal circumstances..

So for example you would expect someone to look sad when talking about sad things, to laugh in response to a joke, to look happy and maybe wistful talking about a happy memory, look concerned when told something worrying, etc., but at the moment her reactions seem to not be the full range of what you would expect, and she doesn't react as strongly to things as expected.

Insight in this context is usually about realising that you are mentally unwell, and that it is your illness which is causing various problems (as opposed to some outside force which may be seen as the source of the problems), though it might be about some other aspect of the part she's playing in what's going on.

So basically it seems like it's saying that she's distressed and nothing much seems to move her out of that particular emotional state, and that she has some but not perfect understanding about why she's so distressed.

Thank you - does it mean that her distress isn’t as evident as you’d expect from what she is talking about? That’s how I read it

It could do. If someone's got a restricted range, then they're not necessarily going to look that different when they're talking about extremely distressing things than they do the rest of the time.
