NC as very outing. We are desperate. A relative is in a complete physical and mental crisis but refusing to go to hospital or engage with the mental health crisis team. Late 20s, physically quite fit, but have become alcohol dependent. Drinking approx 60 units per day but have now cut down to less than half some days. We believe there is a serious mental illness undiagnosed and untreated aside from the alcohol abuse and the patient does accept this but they have said they won’t engage with help because they believe suicide and alcoholism to be their destiny (there is family history of this sadly). They talk about suicide and how they will do it. They haven’t eaten more than a few mouthfuls of food for about 3 weeks. Not drinking any water. Currently on about day 3 of severe alcohol withdrawal. We believe they are suffering delirium tremens and/or are delusional and don’t understand the danger they are in. They are appearing to fit violently several times per day, as in shaking uncontrollably and have hit their head on a hard floor in doing so. They basically need round the clock watching which is untenable for the person they live with. They are talking nonsense a lot of the time, about things at that aren’t real like monsters. We have tried help from mental health team/GP/ambulance service but the moment they hear the word alcoholic their response is that there’s nothing they can do without the patients consent, which of course is not forthcoming. All agree that they need to be in hospital, they just can’t get them there. Every time the ambulance crew have come their diagnosis is ‘drunk’ because they are not equipped to deal with severe alcohol withdrawal (understandably) but because the vitals are within normal range and they haven’t actually attempted suicide there’s not way to get the police to restrain the person in the ambulance. The mental health units advice was that he was in dangerous withdrawal, very poorly, and we should take him home and phone 999. Alcohol service won’t engage until the person approaches them. I understand the reasoning for it all but when you are watching a loved one slowly killing themselves it doesn’t make sense. And we are going round an round in a circle of hell with each service pushing us onto the next. We are told that they won’t section because he is drunk. But I honestly believe he doesn’t have full understanding of the danger he is in, and think he will end up dead without forced hospital treatment. I know without hospital treatment delirium tremens have a high fatality rate. Not really sure why I’m even posting. Just wondering if anyone else has been through similar and if anyone has any wild ideas we haven’t tried or advice.
