Breathing Pattern Problems / Controlling Breathing Constantly

I have had some health issues over the last 4 months, including dizziness and palpitations. I read online that improper breathing could cause dizziness so I started to control my breathing.
This has completely escalated and now I think about every single breath I take. My breathing never seems to be automatic. I wake suddenly at night with my heart racing, night sweats and weird juddering eyes when they're closed and at 4am this morning I woke feeling like I couldn't breathe and if I didn't actually concentrate on taking a breath I wouldn't actually breathe.
I've had this feeling a lot lately and when I'm sitting down in a chair or in the car I feel like I can't actually get any kind of breathing pattern started and end up just kind of holding my breath and taking big breaths. A lot of the time I feel dizzy and lightheaded. Breathing through my nose also feels weird like my breath isn't going to the right place. It seems to hit my throat and make it dry and cold. My neck and left trap area are constantly sore.
Had anybody else experienced this? I'm having a lot of tests at the moment and my GP is struggling to pinpoint any cause, apart from anxiety. He's given my some diazepam which do help a little but I'm still completely hung up on my breathing and manually controlling it.

There is a book called Breathing Pattern Disorders which was recommended to me. I have mild moderate lung disease so I shallow breathe and my neck muscles do more work. Any hoo, the best exercise from the book was to take a bag of rice, lie on your back, place it below the navel and breathe in and out with the aim of lifting the bag of rice, it strenghtens the diaphragm. Really worked for me.
