Care coordinator?

My psychiatrist has said it might be useful for me to have a care coordinator but I’m not really sure what one is..... I had a CPN when I was younger - is a care coordinator the same thing?

If you have one have you found them useful? I’m struggling to leave the house at the moment so I’m hoping that’s something they can work on with me.

I have a care coordinator.
She checks on me every week (mixture of face to face, video call or on the phone) and then talks to my psychiatrist or anyone else I see on my behalf to feed back and equally they all feed back to her. If I need a meeting with anyone she can sit in and remember stuff that I might not.
It means if I have an issue I can go directly to her and she’s aware of all the pieces of the puzzle, so to speak.
I find it a good thing overall.
Ps I also struggle to leave the house so sometimes she comes with me.

Pps they’re not a cpn exactly, their backgrounds will vary. I’ve had a few over the last whatever years.

I have one too, we email every day (well, most days) and she deals with all the letter writing and links between physical and mental health, updates the GP surgery and protects my space to deal.

Plus, she can calm me down in a crisis too.
