Anyone got any experience of severely depressed teen making a sudden recovery?

I’m feeling rather spooked by the last couple of weeks with ds ... for nearly a year he’s had extremely low mood, he’s spent most of the day in bed staring at the ceiling. He dropped out of school (Y13) after the first week of term and since then has barely left the house. He’s not been washing without a lot of prompting or eating properly - he’s always been tall and thin but became positively skeletal. He had three sessions with a therapist at CAMHS in October/November but got discharged as soon as he hit 18 and since then has been passed from pillar to post. He’s been on fluoxetine since CAMHS. Eventually the adult mental health assessment team came out to see him at the beginning of April and they were quite shocked at how low he was and how little motivation he had. The assessor and consultant are coming to see him this week at home, however they’re going to find a completely different boy...

Ten days ago he came downstairs and asked if he could have a phone as a couple of his friends from school had been in touch via Steam (online gaming) to ask him to join their WhatsApp group. He came put with me to buy a phone which was also unusual. Then ds1 asked him to go out last Saturday evening to watch a rugby game and he agreed - again unheard of! The next day he was in bed most of the day so I assumed his burst of activity had been a blip. But then I came downstairs on Monday to find that not only had he got up at 6, he’d also gone for a 5k run in the rain Shock. Since then he’s taken himself off to get a haircut and even gone out to a video game tournament being held the other side of the city. He’s been getting up in the mornings and last night made dinner for everyone without being asked. He’s also eating much better and has put on 3kg.

I’m delighted that he’s obviously feeling so much better but also a bit confused... has anyone else had this with their teen? He’s not able to articulate what’s going on inside his head (he has ASD which complicates things too)

OK i would love to say sudden recovery, i spent a couple of years working as a mh support worker. We were prob worse than our clients most days but i digress.

A couple of things jumps out at me, sudden burst of energy and participation in things, i hate to say it but watch out for suicidal ideation / behaviour on the other hand, he maybe doesnt know it yet.

In some people there can be a feeling of feeling good, back to normal in the days before they complete suicide. I hope this isnt the case here.

He could have a mood disorder such as rapid cycling mood disorder such as cyclothermia www.webmd.com/bipolar-disorder/guide/cyclothymia-cyclothymic-disorder#1 or bipolar disorder / manic depression although this is the first "up" you have seen.

Prozac can take at least 6 months to kick in and do its thing, www.headmeds.org.uk/medications/58-fluoxetine/use_and_action so taking the assumption he turned 18 in November we are just over the 6 months mark, he would have been started off on a lower dose when he was under 18. As soon as he turned 18 he became immediately able to tolerate an adult dose,

The sudden interest and intense interest in things would correspond would serotonin level, levelling out for him with potentially some mania to watch out for due to the sudden swing.

i do hope its not the first option. I say keep an eye open, when i was that age myself i was at my most harmful to myself when everyone thought i was coming back to myself and then bham, the attempt would come, as you can see i didnt succeed.

But there was the everything seemed back to like before illness but that was the calm before the storm, the making memories, i feel horrible about it now but was in the grip of an illness and if it was something that is better understood like cancer i wouldnt beat myself up so i dont anymore.

Pm if i can help
