Where are you on this stress assessment scale? Is there a UK/NHS equivalent

I have found this stress assesment scale and then 1-4 phase outline helpful (need to scroll down a bit for the Q&A) from Canadian MH Services. I am 17 and then phase 3/4. Is there a similar UK/NHS index?

I'm 19 and am 3 (I think?) but I would say until recently, it would be 4 (thankfully getting help, so brought it down). Very useful and interesting.

I am currently on fluoxetine (previously on Sertraline, which didn't seem to work) and have started family relationship therapy (which lowers my stress at home and means I'm less stressed at work too). I've also started exercising more regularly which has helped with my stress and anxiety levels.

I'm a 19 and level 4. But I've not been able to work for a long time. I think the work environment definitely worsened my stress levels.
