severe depression medication wearing off during the day

I've got a chronic health condition that has left me disabled as well as cfs/me, depression, anxiety and ptsd around anything to do with hospitals.

I'm on 60mg of fluoxetine and 50mg of nortiptilyn (although the latter is for pain). I've just completed an 8 week course for pain management which included cbt and relaxation techniques. The problem I'm finding is that the course which was a total of 40 hours at the hospital has triggered my ptsd even more as I was surrounded by all of the reminders of bad experiences in hospital.

The 60mg of fluoxetine was keeping the depression at bay although not getting rid of it. What I've found in the past couple of weeks is that it is wearing off by mid afternoon. The clinical psychologist who ran the course will be recommending further treatment through my gp but I need to get my gp to do something first. He has previously said I need to reduce the fluoxetine before I can go on something else. Is this true? I'll be in an even worse situation then.....

Your medication will be in your system 24/7 so your mood must be changing towards afternoon and it cannot be the medication wearing off unless you are cutting right down.

There are certain treatments for PTSD that require you to not be on medication I haven't heard of Flouxetine being one of those medications I am unsure if it GP or psychologist recommending you cut down, if it is the psychologist I would talk to your GP first.

Sorry my gp said I'd need to titrate down on the fluoxetine as I was on the highest dose so that I could start a different ad.

Ah yes, that's normal when cutting down for any medication, you wont necessarily feel worse if you cut down very gradually, then when you are down to maybe 20mg they may introduce the new medication.

Can I ask what it is they are thinking of giving you to replace fluoxetine, some of them cannot be taken together but some are okay.
