My daughter's temp is 40.1, I've given Calpol an hour ago but it's still high. Does anybody know if I can now give ibuprofen or do I have to wait another three hours? She's 5.

That is a very high temp after an anti-pyretic (anti-fever) medication, and I would call 111 for advice or just take your daughter to A&E. Hope she feels much better very soon, poor thing Sad

You can give her inbuprofen now. It will be much better at getting the temperature down.

It's now 41.3, I've just given the ibuprofen. I've never dealt with a temp this high. Thanks for advice.x

You can give ibuprofen 2 hours after paracetamol, but just make sure you don’t give more than the recommended doses for each in the 24 hours. Try cooling her with cold flannels and cold drinks or ice lollies, but please call 111 if her temperature doesn’t come down. Paracetamol never worked with my son, but nurofen did.

You can give ibuprofen now and, if needed, continue to give both in a staggered pattern provided you stick to the rules of each individual medication (so not more than 4 x paracetamol doses in 24 hours or 3 x ibuprofen in 24 hours or whatever it states).
Ibuprofen often works better on my DC with very high temps. If your daughter's temp stays high 20-30 minutes after giving ibuprofen, I'd seek further medical advice.

Without being patronising, note down the times you give each medication so you can track overnight what you've given and when (maybe it's just me, my maths isn't great at 3am Confused)

I hope she feels better very soon.

Nurofen and Calpol together was the only thing that kept my DC's temp below 40 during covid. I do find calpol doesn't work as well as nurofen does on temps anyway. Give 111 a call if your worried though.

I’d phone 111 regardless of the advice here, they talk you through counting her breaths and things like that to see how she is, it’s easier to get proper advice over the phone no matter how knowledgable people are here. Hopefully the temp comes down soon

The advice I got from a consultant for high fever was: If temp lowers a little after calpol then give nurofen after 2 hours in the case of very high fevers. If calpol doesn’t reduce temp at all, then give nurofen without waiting 2 hours. However, this was the advice given to me after an overnight stay at hospital when my DS had tonsillitis and his fever wasn’t reducing. You may want to check with nhs 111 first.
All 3 of my kids are known for very high fevers (40C) that sometimes don’t even budge with both calpol or nurofen, I know how scary it is. Hopefully your DC will feel well soon. Flowers

Also, is she able to have some fluids? A body raging with temperature has increased metabolism.

Don't let her cool down too quick. Definitely try ice lollies to start with. She will probably need to be checked over though by a health care proffesionl

Poor thing! The ibuprofen should kick in soon and hopefully the temp will come right down.

Nurofen for mine was definitely better for high temps than calpol. Both my children suffer from high temperatures with bacterial infections needing antibiotics (tonsillitis, uti etc). Nurofen is OK for a day or two but 111 should help you with an out of hours GP. They should get you an appointment with that temperature in a child

Thank you everybody for your help. We are off to hospital now, her temp is 42.5 still and her hands and feet are like ice, she is alert and chatting but just very lethargic.

I was hopping on to suggest nurofen but see it's moved on fast. I'm glad you're getting seen quickly, when my DS had a temperature that high he was kept in for a couple of days until it stabilised. Hope you don't have a long wait Smile

That's too high OP. I'm easy with temps but 40 and over you can't get down it's a hospital job. Glad you're doing in.
