NHS online form to accept/decline vaccine for 12-15 not fit for purpose

Anyone else having issues with this sodding form? Our school says it's the only way to accept/decline the vaccine and you MUST complete it before 30th Sept.

I spent all yesterday trying to get into it at all - no joy. This morning I got in - hoorah! - but now I can't get past the name of the school. You have to put in a code (provided by the NHS) and then you get the dreaded spinning wheel, which eventually times out and leaves you exactly where you were before. I've wasted an hour already this morning.

My friend, who got a bit further down the form yesterday, says the same happens when you get to ethnicity and gender (why the f* we have to give this info I don't know - it certainly shouldn't stop you completing a form that has no bearing on either). Argh!!!!!!

I’d guess there’s an awful lot of people trying to use the same site at once. Maybe try late at night or early?

Data capture might be the only way to accept but it's sure as hell not the only way to decline the vaccine. If you're declining, tell the school and to be extra certain they won't be shamed into it, home school them on the day.

It's volume of traffic to the site. Give it a couple of hours and try again.

I've heard nothing from our school yet about the covid vaccine, but considering they managed to fuck up my dds flu vaccine I'm not holding out much hope 😒

I'm definitely NOT declining. But making everyone accept/decline on a form that isn't able to cope with the utterly predictable volume of traffic strikes me as a spectacular own goal! Many parents don't have the time to waste, so lots of DC won't have the requisite permission to allowed them to get vaccinated. What a shitshow!

the information about race and gender is probably so an eye can be kept on demographics with low take up.

I haven't had a problem, but it sounds like we were sent a different form.

It's from my county's community NHS trust @kimlo. I just wondered if other counties' forms are as inept as mine!

I have heard nothing from school, LEA or local NHS Trust. My child has never been offered a flu vaccination either. I think that makes my local authorities more inept than yours, OP, wish they'd bloody hurry up!

My child was supposed to have the vaccine via the GP before he went back to school and it's still not happened. We have heard nothing via the school route either. The system with vaccines for vulnerable children has been a complete cock up as it always is for anything to do with SEND. More bloody chasing as usual.

Not done through school here, just take the child along to a drop in centre accompanied by parent

It is the "schoolscreener" site? I've refused to even create a profile on the online site. If you read the small print of the T&C (it's always in the small print, isn't it...) the site with the consent form is nothing to do with the school, and furthermore by registering you are releasing them from liability for any errors they make. Like...

"Oops, we mixed it up and thought you had given consent, so vaxxed your kid anyway. Oh well, you can't touch us and it was for their own good anyway you filthy anti-vaxxer"

I have finally completed mine. I got stuck on the medical page, all I could see were red lines, no where to answer yes/no. Took me quite a few attempts, was getting rather annoyed with it!

A paper form would be very welcome! And no, it's not the schoolscreener site. This is from our local community healthcare group and we had another email from the school yesterday saying that the site is being taken offline to try and fix the problem. Let's hope a paper form is forthcoming. Sometimes the most technical solution is not the best one!
