Hey guys

So I'm trying to sort out a sick note on the nhs website but I am having a real big problem.

I am a mother of 2 children with asthma with my youngest having chronic asthma. We haven't received a letter to say we have to isolate but as a mother I am doing everything to prevent any of my children catching the virus. I work for the nhs on a gastro ward but we are now looking after 17 covid 19 patients. I am unable to go to work as I fear bringing it home to my children.

Can someone please help me with a link or guide me to the right direction to fill out this form

Unfortunately for my partners mum, she received a letter stating she cannot have any contact with the outside world(so to speak) for 12 weeks. I won't be able to send my children there. As for social services being involved that won't happen as my mother in law is able to do everything herself(most things) but she gets very tired.

Ok, the GP has advised you to stop working due to your child’s asthma. Do they believe your child should be on the shielding list? That’s the avenue you need to pursue, not a sick note, as they are only for use when you’re ill yourself.

Hey, NHS manager here, coincidently, gastro is one of my services.
You can't get a sick note as you're not sick. Your manager and Occy health should conduct a risk assessment with you and look to mitigate the risks of exposure to you by looking to redeploy you somewhere safer (admin or switchboard, or give you paperwork to do in a safer area) or be allowed to work from home doing something akin the above. I have some nurses (from band 5 through matrons to head of nursing) working from home in complaints responses/72 hr reviews/audits/cqc preparation. Ask if that's an option.
If none of that will work you have to ask for annual leave or unpaid leave. They don't technically have to grant either but you are within your rights to ask.
As someone else has said, the children should be going to school as part of key worker access. They're not shielded so the gov don't feel they are at risk which you may disagree with, as is your right, but unfortunately work don't have to give you paid time off! It's a horrid situation.

I know it's on 111 site as husband did one. You have to go through online questions then it asks if you want an isolation note for employer near end

Thank you for you response. That was very kind of you. Atm moment I have had to take 2 days of as emergency leave. My ward manager is off himself and won't be back till Tuesday. I have spoke to occupational health yesterday and they said that they will speak with HR and see if they can redeploy. I hope this will help. I love my job and everything I do to make a difference. I don't like being stuck at home and doing what I need to do to help out. It's not in my nature. But when you have children it's difficult. A shitty situation. I do hope that they can help me out so I can feel like I'm doing something. I also spoke with the children's school yesterday and because off there health problems(both asthmatic and eldest daughter having a bleeding disorder) they said they don't want me to risk travelling 45 min to the school on public transport. Plus I do 12 hour shifts as you know and won't be able to collect them. It's a dad situation to be in.
