Please help, 1 year old with temp of 40!!

something Was telling me to check on my 14 month old an hour after I put him to sleep, he felt red hot and his temp was 40, it’s been 20 mins and is still 39, is this normal?

He’s been a bit up and down the last few days but I’m so sure that’s because of his teeth.

I would wake him and give calpol, if no better in an hour or two nurofen. If he is struggling getting it down call 111. Hope he's ok, nothing worse when our children are poorly.

Sorry I forgot to say I have given him
calpol 20 mins ago, well more like 30 mins ago and still not going down much, is 39

Agree, if you have nurofen given that in an hour or 2 then more calpol when it's time (4 to 6 hours after 1st dose,,) it has come down which is good.

Give it another half an hour / hour. If you can, leave him in just a vest or pull his covers back.

I would make sure that he isn’t overdressed and give nurofen in an hour or two. Then alternate calpol every 6 hours and nurofen every 8 hours to keep him dosed up. Make sure you keep his fluids up too. Check on him regularly throughout the night and if you’re at all worried call 111.

Don't wake him to give meds. Sleep is the most important thing. If he wakes uncomfortable alternate Calpol and nurofen every 2 hours. Temperatures are natural - his body is working as it should do, to fight off an infection. Take care x

Okay I know I’m over worrying but does his skin look slightly mottled to you? Or is this normal? It’s just his feet are cold but rest of him really hot, I heard this was a sign of meningitis

That does look like mottling to me, which can just be viral but of course we all worry about meningitis. I’d call 111 or OOH, you’re not being over cautious.

Ring 111 feet being cold can be a sign of something not quite right! It can also
be nothing! My DS2 ended up in a and e with very similar recently. He had a high temp and cold hands and feet! He was absolutely fine! They got temp under control and was a virus but always best to get checked

Skin could be like that simply because he's got a fever and is running hot, but call 111 to be sure OP.

I don’t think it looks like mottling and my DS often gets cold feet with a high fever but nobody online can tell you what is wrong or reassure you. You are his mum and you know him. If your gut is saying something is off, go with it. When mine are poorly they often look awful with a fever but an hour or so after calpol/nurofen they usually pick up and I start to worry much less.
Sometimes useful to reassess once the temperature is down.
I would say however based on the information you’ve given us 111 will not be terribly helpful and will most likely direct you to A&E.
Hope he’s alright, it’s the worst thing in the world when kids are ill. I find it so worrying and stressful.

A&e if you're worried. Trust your instincts

You will have a long wait on 111. You could see if temp goes down with nurofen and if skin is normal as temp adjusts. Doesn't need to go back to 37 but should be down to at least 38 from 40 after calpol and nurofen.

We had loads of high temps over 40 and freezing hands and feet with purple mottled skin. Soon learnt it was just normal for ours when they got Ill. I spent ages worrying too at first. Personally I'd try not to give too many meds. Temp will fight of bug way faster if you let it.

As worrying as it is, it's common. Most likely a virus

Make sure he's drinking/ wet nappies because dehydration will be when you need to worry

Try to resist A&E for a high temp

Has he had a lot to drink today with properly wet nappies? You need to make sure he is well hydrated.

That's not mottling.

Babies often get cold hands and feet with a high temperature but you are right that it can also be a symptom of meningitis.

Give him ibuprofen if the paracetamol hasn't helped.
If your really concerned then go to ooh or A&E.

Please don't give alternate paracetamol and ibuprofen every two hours, that is far too many doses in a 24 hour period.

To clarify re paracetamol - every 4 to 6 hours, 4 doses in 24 hours
Nurofen - every 8 hours, 3 doses in 24 hours.

I was advised to give calpol, nurofen 2 hours later, then calpol again 4 to 6 hours after 1st dose. Then spaced correctly thereafter.

It doesn't look like mottling to me.
1 of my boys has been mottled when he's held a high temp before and it's more with blueish and purplish type of webbing/lace pattern under the skin.
Thankfully neither times it has been nothing too sinister but it has happened due to tonsillitis. I was then also told because I am a red head myself.. this kind of thing is more common for us - and apparently to our children (my children are blonde!)
