anti-reflex meds are not working, callum is throwing up after every feed...

he isn't going to gain as much this week.... what should i do??

We had this problem with ds, and after every feed we had to keep him upright for half an hour. Can you do that?

I had this with ds1 until he started eating solids. Nothing I did made any difference. Most mums wonder around with a dainty muslin on their shoulder - I had a bath towel! I tried feeding him less, feeding him slowly, keeping him upright - nothing made any difference. But he did gain weight, so he was obviously getting whatever he needed.

Are you sure he's not gaining weight?

Have you increased his top-ups sparkly? Maybe cut back a bit?

What meds is he on - do they need increasing because he's gained (woohoo!) weight?

We also had to keep stopping him during the feed for burps- that worked in combination with keeping him upright. If he took a load with no burps inbetween he was guaranteed to throw the whole lot back up.

dd's reflux never really improved with the tube sparkly - It just aggravated it by keeping the valve at the top of the stomach slightly open also it can make the gag reflex a bit sensitive. I've had an ng myself so know that it can feel a bit strange.

Oh SGK,Poor you. and ds.dd had terrible reflux and would projectile everywhere.(I now realise that as well as reflux this was probably due to her SN and poor gag/swallow reflex and associated issues).I tried the winding thing and often had to have her upright for a good hour,poor ds was somewhat neglected as I was ALWAYS feeding/winding/comforting a screechy baby.It is so hard when they are refluxy.I found a sling helpful,also raising the cot matress by placing a pillow or two underneath,when she eventually deigned to go in a cot at 3 months rather than sleeping over my shoulder.I found it did improve when she got to be able to sit supported about 6 months.What meds is he on?Gaviscon and ranitidine are the usual ones,how long has he been on them?

he is on rantidine and domperidone, i did worry if he needs a bigger dose now he is finally gained weight

yes he is finally gaining weight! I have been puked on all day, every time I have winded him, he has just thrown up all over me.. he even was sick on Dd1 earlier.

SGK - I know things are more serious with you, given that Callum was a preemie. But even with me and ds1 who was 3 weeks early (so not technically a preemie) no-one was in the least bit worried (apart from me) that he kept chucking up everywhere!

The HV kept suggesting various different things, none of which worked, and it was really just one of those things were we just had to keep going and eventually it stopped. Presumably he just grew out of it.

The good thing is that he is gaining weight. As long as he is feeding and seems happy then I would say that things are going well!

the problem is taht he wasn't gaining weight and we had to start topping up via a NGT tube, SCBU nurse threatened us with formula topups (which we have refused to do) and he actually gained 9oz last week. SO if he doesn't gain well this week, we will have the SCBU nurse on our case again..

But he gained 9oz with you breastfeeding? That's great! Just because he's throwing up doesn't mean he won't be gaining weight.

I know that this sounds really patronising - and I know that I wasn't in your situation - but try and relax and just get on with doing what works for you/what you feel comfortable with. All babies throw up to some degree.

If Callum seems happy, and feeding is going well, then just relax and see what happens next time you go for the weigh-in.

SCBU is probably making things worse for you all rather than helping (although I'm sure his/her intentions are good)

sparkly - have you checked his tube placement this evening? - if he's been sick alot it could have shifted a bit and be making him uncomfortable. dd used to vomit the end of hers up. The problem is the only solution is to pull it and reinsert. Are you doing that yourself?

the tube was put back in yesterday, he has pulled his tube out, so I had to put in a new one. I'll see if it that.

SGK - hope you can settle him soon. Now look - I still think he will gain - but if he doesn't then just tell the SCBU nurse that the reflux has been bad and can she get you a physician's opinion on that and that alone. Tbh I would think the reflux is another arguement against giving formula - last thing your poor mite needs is someone messing with his diet! Also - I only have experience of babies born at term - but for them - some weeks are bigger growing weeks than others - 9oz last week, 3 say this - would still average out at 6oz per week (6 being a magic number eh?) Surely they can cut pre-term babies the same slack? If all else fails just tell the SCBU nurse to be nice or many angry m'netters will be after her!

Actually maybe better not to mention that..........

lol northern.. I will tell her that its been bad and I need to know if the meds need increasing. WHat a shame is that he is throwing up the EBM, today I have struggled with the expressing (not sure why) and to see it come straight back up is heartbreaking

This is the drug formulary that most Dr's work from when prescribing for children. You have to register but it has a lot of info.

I found that my paediatrician had prescribed the dose of Zantac/Ranitidine for a 1kg baby rather than the 6.5kg he was at the time.

Its always worth checking!

PS know about how heartbreaking it is to see milk come up. DH and family just cant understand why I can get upset over (LOT) of spilt milk. I found a dummy a good way of helping it stay down but thats not to everyone's taste.
