Ongoing petechiae-very worried

Hi All,

I am just wondering what would you do .

Long story so bear with me.

2 month ago I found a lump on my 7 years old girl neck and she was complaining of being more tired and had a leg pain mainly in the evening.

We went to our Gp who sent us for an ultrasound and blood test.

Ultrasound came back as reactive lymph node and blood test came back ok also.

A few days after her blood test I’ve realised that she has very few red spots on her body ( mainly neck and back area) but I found on her leg tummy etc .

They were not fading under the glass.

Next day we went to our Gp who said it is petechiae but as she has only a few and had a blood test not a long time ago she is not concerned as it can be from lots of things ( heat scratch etc)

She got few spots every day here and there and they were fading by next day or so but she kept getting new ones.

We asked the Gp again who said she is not concerned.

The spots stopped coming for 2 weeks and I was so relieved however this Monday when she had around 40 on her shoulders , no fever no sickness and keep getting few ones every day mainly on her back and neck .

Next day the same so we went to the Er.

The doctor looked at me with a very big open eyes saying that they are petechiae but this is not the kind of petechiae they are worried about.

However he ordered for blood which came back ok, however her haemoglobin was borderline.
( the blood test a month ago was in the morning that one was in the evening so they can cause the differences maybe )

I honestly do not know what to do at this point.

Should I push for specific blood tests? Should I go back to my Gp to refer us somewhere else?
Or should I be calm that her full blood count came back ok?

Did any of you were in similar situation? What was the outcome?

I am honestly very nervous.

As long as they were in normal range it’s fine. They might change slightly depending on time of day. Keep an eye on her. Any changes take her back to GP or A&E for another blood test.

The first blood test her red blood cell was 4.95 at the Er 4.4 and her white blood cell at the first time was 8.8 at the Er was 10.4.

do you think it’s a slight change ?

May I ask you how long was the getting these petechiae spots?

we are in a second month 😔

They only first appeared about 3 weeks ago. I haven’t seen her for two weeks as she’s gone abroad so not sure if she still has them.

I have this on my legs and feet - it tends to come up with pressure and heat it does not always have a serious cause - they will have tested kidneys, liver, possibly cardiac enzymes as part of her blood tests. If her FBC came back ok then thats a good sign - her Haemoglobin could be a little off if she was a bit dehydrated for example.

If you are worried go back and see the GP.

You seem to be very anxious.

If your daughter is otherwise well, try and accept the clinicians reassurances.

Obviously if more symptoms emerge reassess.

I have these as an adult and always had them. I have low iron and my platelets fluctuate. I bruise super easily and quite tired most of the time.
I’ve had loads of tests done and it’s just how I am unfortunately

when you say heat do you have to stay on the sun to cause petechiae or when let’s say it’s warm in the house do you also get them?

the first time it was 8.8 and at the er it was 10.4

also her red blood cell at the first blood test was 4.95 at the er it was 4.4

the first blood test was in the morning the second one around 8pm at the er.

inwas wondering since if it’s still normal …

I get them on my shoulders if I’ve been carrying something heavy in a shoulder bag. A bit of a long shot but has she been playing games where she carries things in a backpack?! Is her school bag heavy?

No she gets on random places every day 2-5 new ones mainly on her back and shoulder

Yes so it tends to be heat and pressure off my shoes - or if my bra has rubbed a bit. I have them on the bottom of my feet too. I had them last summer and this year I had them early June (still here). Lots of blood tests done - all ok.

Had she been wearing a backpack those days? I get these if I’ve been carrying a bag or shopping bag on my arms, where the handles dig in to my skin. DD also gets petechia around her eyes when she’s cried a lot the day before. They can be quite common.

Being tired and aching legs could be growing pains. DD was complaining of pains in her legs and was tired, the doctor said as it was of an evening they weren’t concerned as that’s more a sign of growing pains, if it was happening all day they’d be more worried. After a month or so we noticed it was more on days when she had done a lot of pe and after school sport. I was probably too quick to go to the gp about it but DHs cousin had childhood leukaemia so I was a bit worried.

Hiya I just checked on the North Bristol NHS Hospital Blood ranges (I am presuming you are in the UK 😀) for children RBC normal is 4-5.2 and WBC is between 4.5 - 14.5 thats for 6-12 year olds. There will be some slight difference as some hospitals use slightly different reference ranges - but it won’t be much. Your daughters results are way within the normal ranges. Blood results can change based on time of day, hydration levels etc - but if you are worried talk to the GP again.

I've suddenly come out with loads of these in the last couple of months - had a blood test that all came back normal, so I'm just planning on altering my diet. They're not interested in investigating further, as I'm otherwise well. 😊

What are you actually worried about? Doctors don't mess around when it comes to kids and potentially serious things. It sounds like she's had all the appropriate tests and the doctors aren't concerned. If she's well in herself, I would try and enjoy the summer!

tbh she needs to be seen by a paediatric doctor. May I ask if you went to a specialist paediatric A&E? because if not... that would be the next step I would recommend. or see a private paediatric haematologist/rheumatologist... or even a general paediatric doctor

We went to the Er but I don’t think the doctor was a paediatric doctor to be honest 😔

we are going to see a paediatric haematologist in September.

@MimiDuncan I will be honest OP, my knowledge of paeds is limited to anaesthetising them and treating them in emergencies... so not the best person to give advice. also Internet not the best place for medical advice.... my baseline on social media is always to recommend face to face medical advice and children are very different to adults so paediatrician would be my go to. depending on where you live, there should be a childrens hospital... and they will have an A&E. you are the mum and as such the expert on your child. if your gut is telling you your DD needs to be seen before September, go to the paediatric hospital A&E or see a paediatrician

I feel that doctors are missing something.

they we’re not paediatrician, it was a Gp and Er doctor and they were not investigating at all.
