To go to A&E with hemerroids

I am 37 weeks pregnant and have hemerroids, I can't get a proper look I think there's 3 and they are swollen and angry and agonising. I can barely walk or sit. I phoned GP and got scheriproct ointment and suppositories which I'm using but I have never been in so much pain in my life and nothing is giving me relief. I've been lying in bed with ice on them for an hour just tried to stand up and it's still agony. I'm weeing a million times a day and night and when I sit on the toilet and then stand up they sort of contract every single time and it brings tears to my eyes how much it hurts. The gp never looked just prescribed over the phone. I'm worried something is wrong I've had piles before but this pain is something I've never experienced 😭😭😭

Sympathies, it is the most terrible pain ever. I once ended up in out of hours where a Gp sent me straight to a hospital surgical assessment unit, not that they were very helpful there. they said they would put me on a list to have them surgically removed but I never heard anything after that and have not chased it up. There is an ice pack type thing you can buy in the shape of a sanitary towel which can be put in the freezer and then applied which might bring some relief. It's a worry as labour could make them worse. On another recent thread on the same subject people were saying a GP could pop a thrombosed haemorhoid in the surgery but in the few times I have gone to a GP when they have bene excruciating, I have never been offered this.

@Yesthatismychildsigh that is all really irrelevant. You were not pregnant and the OP is. Treatment pathways are completely different, they will not risk an anesthetic for piles!

I don't think it's that simple when you're pregnant. But oromorph would be appropriate for the amount pain it causes!

@Mars27 Gastroenterology is a branch of medicine, and Colorectal is a branch of surgery. They are different things and deal with different issues.

I would be truly astonished if the on-call surgical team would see a patient with thrombosed haemorrhoids, especially on a night shift. In nearly a decade of practice, I’ve never seen it happen.

OP, please call 111, this is what they are there for. If you go to A&E, you will most likely have a long wait on a hard chair to be sent home with some painkillers, whereas hopefully 111 can get you into an out of hours service much faster.

A&E is not the place to go for a ‘fast-track’ referral for a non-emergency operation. My trust will only do referrals for genuine emergencies, or for two week waits for suspected cancers. Referrals for haemorrhoids would need to be done via your GP.

I do have sympathy for the pain you’re in, and it must be awful, but I really don’t think sitting in A&E will fix that. If you do go, take a snack, a book, and a phone charger, and prepare for a very long wait.

I was seen on a surgical day unit for this many years ago, admittedly not at night but was referred straight from the GP as in so much pain.

@Mars27 colorectal is not part of Gastro, Gastro is medical not surgical. The OP should contact out of hours and explain her problem not just go to AE where she will probably be sat for hours and hours in the wrong department.

I work in maternity.

I would call 111, they can either “book your arrival” at A & E if the triage deems that necessary (your pain sounds severe which would be A & E, where you can be admitted for pain relief and make sure baby is happy) or they can book you an ooh gp appointment who may well send you in anyway.

Hope you get seen and are more comfortable soon.

are you a midwife or a doctor? because I can’t think of an a&e in the land that would ‘book’ a hospital admission for piles, nor one that has time to pop on a CTG to ‘make sure baby is happy’ because of…piles

There is no requirement that a condition has to be life-threatening before people can be seen in A&E.

My role isn’t relevant.

Our local 111 service can book arrival time appointments at A & E, it’s offered to anyone who they refer there, obviously emergencies take priority…

and my point about ctg monitoring was if the OP needed stronger pain relief it is an option to be admitted so we can monitor baby while pain is managed, it happens on a daily basis, albeit not commonly for piles.

Oh OP I really feel for you. I had exactly as you're describing after my first delivery and then during, and after, my next two pregnancies. It was why we had no further children. That spasm of pain...well I still shudder at the memories!

I was prescribed the same as you and it did ease it eventually, but what I did find was that when the spasm comes, trying to push against it stopped it being quite as painful. Not a huge lot granted, but truthfully any improvement when you're at that level of pain is something.

Could you speak to your midwife? It's going to be very painful for you to give birth if they are still really bad. I am sure she would want you to be sorted out now rather than have to sort you out on the day.

This is really grim and I'm sorry but I have taken a photo which short of uploading on here I can only describe as two large hemerrhoids which look like a big fleshy lump, then next to it is a much much smaller pea sized lump that is tight, shiny and dark purple/blue. I think that's the one that is absolutely killing me. Does that sound Normal? I've looked at a million vile photos online but I can't find one that looks like it with the fleshy lumps and the tight smaller very dark purple ones. I'm worried it's something worse than a pile! I don't know anywhere I can upload a photo and get advice and I don't really want to email it to my GP as that isn't anonymous Blush

Initially I was explaining how bad I know it is, sympathising. Secondly I was answering a post that said it wouldn’t happen. I even qualified that by stating that I wasn’t pregnant.

They sound thrombosed and yes same when I had them the smallest one was the most painful. I would ring your Gp first thing in the morning, ask for a face to face appointment to check they are not strangulated. The pain will subside but with me it did take perhaps up to a week.

whilst Waiting overnight I was given instillagel. It helped a fair bit. It’s just gel in a big syringe. I don’t know if you could have it but maybe it’s worth asking anyone you can get to speak to. I really feel for you, sweetheart. Mine were bad enough and I wasn’t heavily pregnant!

Yes I was given a syringe of this when in hospital too to take home. Instillagel and lactulose to soften things. the Instilagel was really helpful.

Yes, they gave me loads to take home. Can totally recommend Huddersfield hospital. It helped me loads, too.

Hope you have pain relief sorted for birth. In the meantime see GP. Insist! Or at least send in photos. They might have a nurse appointment for you.

I feel your pain I got pies when I was 40+1 and I was absolutely raging cos I was like baby should be here and this has happened. Wasn’t too bad to begin with but when I was 41 weeks it was the worst thing I’ve ever been through, my mum had to look after my DD I wasn’t able to sleep ended up in OOH at 1am was just horrendous and wouldn’t wish it upon anyone.
i found warm baths were the only thing that really helped and positive thing was a week after baby was born it was better but it was pure hell and I would choose labour over that any day . Hope it eases for you

Send someone, or if you can…go to the 24 hour shop and purchase pure tumeric powder, from the herbs or Indian section mix it with olive oil - NOT SUNFLOWER oil. Warm it in the microwave to a paste cream…apply while warm.
the pain will go instantly and they will start to go away. Every 4/6 hours.
