Dd injured - not happy with A&E.....

Dd took a crashing fall from a height at sports training last night and has hurt her foot and unable to bear weight on it.

Went to A&E last night and they took X-rays and said her ankle was fine. They wouldn't listen to her when she repeatedly told them the pain was under her arch and across the top of her foot not in her ankle. Sent her home still unable to weight bear with no crutch's and a self care leaflet for ankle injuries.

Would ibu to take her to our local minor injuries unit today for a second opinion? She's on paracetamol and ibuprofen alternating every 2 hours and it's only just taking the edge of the pain. Foot is swollen along the outside and across the bridge and is very painful to the touch. She also has a high pain threshold so when she is crying in pain I sit up and take notice!

Dh reckons they will review her X-rays as a matter of course this morning and will ring if they've missed anything - is this right?

OP I hope your daughter is ok. I’d definitely go for another opinion.

You can alternate them but still have to follow the dosage guidelines about tje maximum allowed in the day.

Is there anything stronger I can buy otc as she been up all night with the pain?

Check with GP. Have been advised before to do the 2hr rotation thing when pain is really quite bad and it does work well - so dep on your daughters age this may be appropriate x

I'd definitely ask for a second opinion citing that the first doctor was not listening to where the pain actually was and that the xray was also of the wrong part of her foot.

And maybe when you have got her sorted, bring it up with PALS that they were not listening to tje patient.

OP didn’t say her dd’s age. My 14 year old is taller and heavier than me.

The misinformation about painkillers is astonishing if she's been taking paracetamol and ibuprofen alternating every two hours you need to take her to be assessed for overdose of medication. Paracetamol is every four hours no more than 6 tablets in 24 hours and ibuprofen is every 6 hours no more than 6 tablets in 24 hours and that's for an adult. (Also paracetamol and ibuprofen can be taken together at the same time, but paracetamol is still every 4/6 hours and ibuprofen 6 hours)
So I'd be taking her to minor injuries for the foot and to have her bloods done for the medication and correct dosage.
(Information is all on the nhs website, search paracetamol dosage and ibuprofen dosage)

The difference between a fracture and a sprain, is in the weight bearing. If you can't put weight on the affected 12 hours after a fall without excruciating pain, it's likely a fracture.

Not all fractures will be detected by x rays. The OP's daughter needs to go back to A&E and ask for a MRI Pain killers will do nothing to fix a fracture.

She could have pulled a tendon or damaged ligaments or have soft tissue damage - I've managed to do all three and they're very painful.
Soft tissue damage, I was given a boot.
The pulled tendon - they just told me to elevate it when not using it and to take painkillers every four hours. Use heat and ice as appropriate.
The tendon took longer to heel than the ligaments and the tissue damage. With the tendon, the pain was under the arch and across the top. A&E made me stand on my tiptoes and walk in my bare feet to assess whether it was my Achilles tendon I'd hurt. It wasn't.

Give her co codamol I've just seen she is sixteen

Try 8/500 and if no relief then try solpadeine max which has more codeine in it

Call your GP and ask for pain killers to be prescribed

I was prescribed codeine and tramadol for my periods I had endo and I was 13 years old.

You poor things

I would absolutely get a second opinion. Ankle/foot fractures are routinely missed - even when reviewed.
I had an injury playing sport. Had X-rays. A&E said bad sprain, and to weight bare as much as poss. 8 weeks later I'm struggling. MRI showed displaced fracture which needed surgery to fix/pin and I'm now left with arthritis because they missed it.

If they miss an injury and it needs to be broken again or her recovery is longer then expected what are the implications for her playing the sport she loves? Make sure you go to a minor injuries I hope you both feel better soon. It's dreadful seeing your child suffer.

18 months ago I went over on my ankle. I wears a crack but assumed that same as always a bad sprain. I went to minor injuries a week or so later as it was still agony on the top of my foot. All ok. Soft tissue, will heal in time. Saw a consultant who asctuakly said that I had broken a bone and it had healed wrong and amongst the ligaments that needed to be repaired I needed the bone re broken and put together properly. The docs can definitely miss things! Also my DD hurt her foot at a sport and she couldn't weight bare at all. She had ripped the ligament off the bone on the side of her arch. She was in a cast/boot non weight bearing for 9 weeks

I think she has a fractured navicular bone from your description. I would take a bet on it.

I believe you, I broke my foot years ago and had the same.

Sheered the ends clean off the bones through the arch (you could actually see the misalligned bones sticking out under the skin with your naked eye) during a bad landing where I missed the crash mat in gymnastics training and yet they kept infuratingly talking about my ankle.

They kept saying 'it looks fine probably just a sprain' and wouldnt listen, when one of them grabbed the BROKEN bones to twist my foot to look at the ankle we ended up screaming at them 'we're not here for the fucking ankle'.

They did eventally xray the correct part, I was in a cast for months and couldnt walk properly for over a YEAR afterwards no appology from the stupid nurse who made it worse through simply not listening or paying attention though.

Ibuprofen for children over 12 years is every 4 hours (it says so on the packet in front of me).

I'm guessing the every 6 hours advice is helpful to make sure you are dosed up when going to sleep as you can only have three doses in total. So something like - 8am, 12 midday, 4 pm and 8pm paracetamol and 10am, 2pm and 6pm ibuprofen - but that would then leave you with quite a lot of time during the night in pain.

When I was dosing like that (post surgery when codeine made me ill) I used to write the times on the packet to make sure I didn't overdose.

Codeine gave me worse pain (stomach) than the actual surgery. I used the laxative stuff as directed. Only lasted about a day before I had to stop. I won't take it again.

Side effects are pretty grim for a lot of us but honestly the pain outweighed those for me on all those occasions I've taken it

It did also make me throw up when I first started to use it. I have to take dihydrocodeine daily and that also is bleak but I can't cope without it

Er, no - codeine is a gift from the Gods! I sometimes take it if I have a very slight headache (any excuse) just to get the wee buzz it gives you! Probably shouldn't admit that but hey ho. It really is the best thing since sliced bread! In fact, better than sliced bread. Although not thick white bread with lashings of lurpak, nothing quite lives up to that.

Do you know exactly what they xrayed? I've x rayed so many people who (knew better than me ) have said I haven't done the correct side , xrayed their foot when it's the ankle they've hurt etc
An ankle X-ray will show some of the foot .
